Esomeprazol ist ein Arzneistoff aus der Gruppe der Protonenpumpenhemmer, der in der Medizin unter anderem zur Behandlung von Magen- und Zwölffingerdarmgeschwüren sowie bei Refluxösophagitis (einer entzündlichen Erkrankung der Speiseröhre) eingesetzt wird. Esomeprazol ist im deutschsprachigen Raum (Deutschland, Österreich, Schweiz) unter dem Handelsnamen Nexium von der Firma Astra (heute AstraZeneca) auf den Markt gebracht worden.[2][3][4] Inzwischen gibt es auch mehrere Generika.
Grundsätzlich wirkt Esomeprazol wie das racemische Omeprazol. Gegenüber dem Racemat ist die Wirkung verstärkt, wobei sich diese Verstärkung durch eine etwas geringere Metabolisierungsrate und damit verlängerte Verfügbarkeit ergibt, während die beiden Enantiomere von Omeprazol selbst keine unterschiedliche Wirksamkeit haben.[5]
Esomeprazol ist das (S)-Enantiomer von Omeprazol (siehe dort) und wird wie dieses in Form magensaftresistenter Tabletten oder als Infusion angewendet.
In Fertigarzneimitteln kommen sowohl das Natriumsalz[6] als auch das Magnesiumsalz (als Anhydrat,[7] Dihydrat[8] oder Trihydrat[9]) zum Einsatz.
Als weitere Salze sind das Strontiumsalz (Anhydrat[10] oder Hydrat[11]) beschrieben.[12]
In den USA wurde Esomeprazol im März 2014 in einer 20-mg-Retardformulierung für die rezeptfreie Anwendung zugelassen. Einen Wechsel in der Verkaufsabgrenzung nennt man in der Pharmaindustrie „Switch“.[13] In Deutschland ist der Wirkstoff seit August 2014 zur Behandlung von Sodbrennen und saurem Aufstoßen in einer Einzeldosis von 20 mg und in einer Tageshöchstdosis von 20 mg für eine maximale Anwendungsdauer von 14 Tagen und in einer maximalen Packungsgröße von 280 mg verschreibungsfrei erhältlich.[14] Vorangegangen war die EU-weite Zulassung eines entsprechenden rezeptfreien Präparates. In größeren Packungen, höheren Dosierungen und anderen Anwendungsgebieten bleibt Esomeprazol weiterhin verschreibungspflichtig.

Nexium ist zugelassen zur Behandlung von Erwachsenen mit gastroösophagealer Refluxkrankheit (GERD) zur Behandlung der erosiven Refluxösophagitis, in der Rezidivprophylaxe bei Patienten mit geheilter Ösophagitis als Langzeitbehandlung und zur symptomatischen Behandlung der GERD.
In Kombination mit einer geeigneten Antibiotikabehandlung ist Nexium außerdem angezeigt in der Eradikation des Helicobacter pylori (Hp) zur Heilung eines Hp-induzierten Ulcus duodeni und zur Rezidivprophylaxe peptischer Hp-assoziierter Ulcera.
Außerdem ist Nexium zugelassen für Patienten, die eine kontinuierliche NSAR-Therapie erhalten, um NSAR-bedingten Ulcera ventriculi vorzubeugen sowie bei Risikopatienten zur Prophylaxe von NSAR-bedingten Ulcera ventriculi und duodeni.
Nexium ist außerdem zugelassen zur Langzeitbehandlung nach intravenös induzierter Prophylaxe von erneuten Blutungen von Magengeschwüren und zur Behandlung des Zollinger-Ellison-Syndroms.
Für Jugendliche ab 12 Jahren ist Nexium zur Behandlung der GERD zugelassen.
Esomeprazol ist das S-Enantiomer des Omeprazols. Daraus leitet sich auch der INN-Name ab.
Es wird im Dünndarm resorbiert und gelangt von dort über die Blutbahn in den sekretorischen Kanal der Belegzellen der Magenschleimhaut. Dort bildet sich aufgrund der hohen Azidität aus dem Prodrug das wirksame Sulfenamid. Dieses bindet irreversibel an das Enzym H+/K+-ATPase und stoppt so den Transport von Protonen in das Magenlumen.
Sowohl das S- als auch das R-Enantiomer haben die gleiche pharmakodynamische Aktivität. Im Gegensatz dazu zeigen die beiden optischen Isomere Unterschiede im Metabolismus. Studien deuten darauf hin, dass das S-Enantiomer in der Leber langsamer metabolisiert wird als die R-Form. Daraus resultiert eine zweifach höhere Fläche unter der Plasmakonzentration-Zeit-Kurve (AUC), was sich allerdings nicht in den Dosierungsempfehlungen widerspiegelt.
Je nach Indikation werden einmal täglich 20 mg oder 40 mg Esomeprazol eingenommen.
Bei älteren Patienten sowie bei Patienten mit Nierenfunktionsstörung oder leichter bis mittelschwerer Leberfunktionsstörung ist keine Dosisanpassung erforderlich. Eine Nexium-Behandlung von Patienten mit schwerer Niereninsuffizienz sollte mit Vorsicht erfolgen. Bei Patienten mit schwerer Leberfunktionsstörung sollte die tägliche Nexium-Dosis 20 mg nicht übersteigen.
Es ist über Interaktionen von Omeprazol mit einigen Proteasehemmern berichtet worden. Die klinische Bedeutsamkeit und die Mechanismen, auf denen dies beruht, ist nicht immer bekannt. Ein erhöhter pH-Wert im Magen während der Therapie mit Omeprazol kann die Absorption des Proteasehemmers verändern. Andere mögliche Interaktionsmechanismen beruhen auf der Hemmung von CYP2C19.
Bei Atazanavir und Nelfinavir sind bei gleichzeitiger Anwendung von Omeprazol verringerte Serumspiegel beobachtet worden, und eine gleichzeitige Anwendung wird nicht empfohlen. Die gleichzeitige Anwendung von Esomeprazol und Nelfinavir ist kontraindiziert.
Die Suppression von Magensäure während einer Behandlung mit Esomeprazol und anderen Protonenpumpeninhibitoren (PPI) kann die Resorption von Arzneimitteln, deren Resorption vom pH-Wert der Magensäure abhängig ist, vermindern oder erhöhen. So kann die Resorption von beispielsweise Ketoconazol, Itraconazol und Erlotinib vermindert werden, die Resorption von Digoxin hingegen heraufgesetzt sein. Die Anwendung hoher Esomeprazol-Dosen sollte daher bei älteren Patienten mit Vorsicht erfolgen und die Digoxin-Spiegel kontrolliert werden.
Esomeprazol hemmt CYP2C19. Bei gleichzeitiger Anwendung von Esomeprazol und Wirkstoffen, die ebenfalls über das Enzym CYP2C19 verstoffwechselt werden, wie Diazepam, Citalopram, Imipramin, Clomipramin, Phenytoin, kann deshalb die Plasmakonzentration dieser Wirkstoffe erhöht werden, sodass eine Herabsetzung der Dosis erforderlich werden kann.
Kopfschmerzen, Bauchschmerzen, Durchfall und Übelkeit gehören zu den in klinischen Studien und nach der Markteinführung von Esomeprazol am häufigsten genannten Nebenwirkungen. Dosisabhängige Nebenwirkungen wurden nicht festgestellt.
Esomeprazol darf nicht zusammen mit Nelfinavir angewendet werden.
Bei Überempfindlichkeit gegen den Wirkstoff oder einen der sonstigen Inhaltsstoffe ist das Arzneimittel kontraindiziert.
In einer randomisierten doppelblinden Multicenter-Studie erhielten 1960 Patienten mit Reflux-Ösophagitis über acht Wochen einmal täglich entweder 40 oder 20 mg Esomeprazol beziehungsweise 20 mg Omeprazol. In der achten Woche waren 94,1 Prozent der Patienten, die 40 mg Esomeprazol erhalten hatten, und 89,9 Prozent aus der 20-mg-Esomeprazol-Gruppe geheilt. Unter 20 mg Omeprazol betrug die Heilungsrate 86,9 Prozent. Nach vier Wochen schnitt Esomeprazol in der 40-mg-Dosierung besser ab als 20 mg Omeprazol.
Eine andere Arbeit vergleicht die Effektivität einer einwöchigen Tripeltherapie (20 mg Esomeprazol, 1000 mg Amoxicillin und 500 mg Clarithromycin jeweils zweimal täglich) mit dem Standard bestehend aus 20 mg Omeprazol, 1000 mg Amoxicillin und 500 mg Clarithromycin zur Eradikation von Helicobacter pylori. Die erste Gruppe erhielt nach den sieben Behandlungstagen über drei Wochen täglich ein Placebo. Die Probanden der zweiten Gruppe schluckten stattdessen anschließend drei Wochen lang täglich 20 mg Omeprazol. Nach vier Wochen lag die Heilungsrate in beiden Gruppen bei über 90 Prozent.
Die momentane Datenlage spricht zwar für Esomeprazol. Dennoch stimmt nachdenklich, dass dieses in allen Untersuchungen mit Refluxpatienten bislang nur mit niedriger dosiertem Omeprazol (20 mg) verglichen wurde. Es fehlt eine Studie, die 40 mg Omeprazol mit 40 mg Esomeprazol vergleicht. Die klinische Relevanz der Wirkungsunterschiede zwischen den niedrigeren Dosierungen (20 mg Esomeprazol versus 20 mg Omeprazol) ist zudem fraglich. Denn auch in Bezug auf Wechselwirkungen und Nebenwirkungen bringt Esomeprazol keine Vorteile.
Nexium ist bei Temperaturen nicht über 30 °C zu lagern. Das Behältnis ist stets fest verschlossen zu halten, um das Arzneimittel vor Feuchtigkeit zu schützen.
Nexium ist verschreibungspflichtig. Daneben gibt es rezeptfrei erhältliche Varianten.

Während der Schwangerschaft sollte Nexium nur unter Vorsicht angewendet werden. Eine Anwendung während der Stillzeit wird nicht empfohlen.

PPIs reduce the production of acid by the stomach. They work by irreversibly blocking an enzyme called H+/K+ ATPase which controls acid production. This enzyme is also known as the proton pump and is found in the parietal cells of the stomach wall.
PPIs treat conditions that are caused by either an overproduction of stomach acid or exacerbated by stomach acid. Taking a PPI once a day inhibits around 70% of proton pumps, so a small amount of acid is still available for food digestion.
PPIs may be used for the treatment of:

All PPIs work in the same way, by inhibiting the proton pump. However, there are differences in their propensity for drug interactions, and in the way they bind to the proton pump. This can affect how long they last for.
All the PPIs available in the U.S. are metabolized in the liver by certain liver enzymes (mainly CYP2C19 and 3A4). There is a lot of individual variation in the way these liver enzymes work, and experts have identified three categories of people; extensive metabolizers (homEM), poor metabolizers (PM), and people that sit somewhere in between (hetM).
Approximately 3% of Caucasians and 15-20% of Asians are PM. PM take longer to excrete the drug and therefore their response to PPIs is increased several fold. For example, the pH of gastric acid is around 6 in PM after PPIs compared to around 3-4 in extensive metabolizers (where 1 is the most acidic pH). Metabolism is also affected in people of an older age and in those with liver disease. Esomeprazole is the only PPI that is well tolerated by people with liver disease. The extent somebody is able to metabolize a PPI can also affect their risk of drug interactions.
When taken at the recommended dosage for the recommended duration of time, PPIs are considered safe. However, they have been associated with several serious adverse effects including:
In addition, there is a risk that the use of PPIs may mask the symptoms of gastric cancer. All patients with a suboptimal response to PPIs or whose symptoms recur following PPI withdrawal should have additional diagnostic testing, and an endoscopy should be considered in older people before treatment initiation.
Patients should only take PPIs as directed by their doctor or according to the instructions on the packet if bought over the counter.

Nexium (esomeprazole magnesium) is a proton pump inhibitor that decreases the amount of acid produced in the stomach.
Nexium is used to treat symptoms of gastroesophageal reflux disease (GERD) and other conditions involving excessive stomach acid such as Zollinger-Ellison syndrome. It is also used to promote healing of erosive esophagitis (damage to your esophagus caused by stomach acid).
Nexium may also be given to prevent gastric ulcer caused by infection with Helicobacter pylori (H. pylori), or by the use of nonsteroidal anti-inflammatory drugs (NSAIDs).

Nexium is not for immediate relief of heartburn symptoms.
Nexium can cause kidney problems. Tell your doctor if you are urinating less than usual, or if you have blood in your urine.
Diarrhea may be a sign of a new infection. Call your doctor if you have diarrhea that is watery or has blood in it.
Esomeprazole may cause new or worsening symptoms of lupus. Tell your doctor if you have joint pain and a skin rash on your cheeks or arms that worsens in sunlight.
You may be more likely to have a broken bone while taking this medicine long term or more than once per day.
You should not use Nexium if you are allergic to esomeprazole or to similar medicines such as lansoprazole (Prevacid), omeprazole (Prilosec, Zegerid), pantoprazole (Protonix), or rabeprazole (AcipHex).
Heartburn can mimic early symptoms of a heart attack. Get emergency medical help if you have chest pain that spreads to your jaw or shoulder and you feel anxious or light-headed.
Tell your doctor if you have ever had:
severe liver disease;
lupus;
osteoporosis or low bone mineral density (osteopenia); or
low levels of magnesium in your blood.
You may be more likely to have a broken bone in your hip, wrist, or spine while taking a proton pump inhibitor long-term or more than once per day. Talk with your doctor about ways to keep your bones healthy.
Ask a doctor before using this medicine if you are pregnant or breast-feeding.
Use Nexium exactly as directed on the label, or as prescribed by your doctor.
Take each dose with a full glass (8 ounces) of water.
Nexium should be taken at least one hour before a meal.
Do not crush or chew a delayed-release capsule. However to make swallowing easier, you may open the capsule and sprinkle the medicine into a spoonful of pudding or applesauce. Swallow right away without chewing. Do not save the mixture for later use.
The capsule can be given through a nasogastric (NG) feeding tube. Read and carefully follow any Instructions for Use provided with your medicine. Ask your doctor or pharmacist if you do not understand these instructions.
Esomeprazole is usually given for 4 to 8 weeks only. Your doctor may recommend a second course of treatment if you need additional healing time.
Use this medicine for the full prescribed length of time, even if your symptoms quickly improve.
Call your doctor if your symptoms do not improve or if they get worse while you are taking this medicine.
This medicine can affect the results of certain medical tests. Tell any doctor who treats you that you are using esomeprazole.
Some conditions are treated with a combination of esomeprazole and antibiotics. Use all medications as directed.
Store at room temperature away from moisture and heat.

Use the medicine as soon as you can, but skip the missed dose if it is almost time for your next dose. Do not use two doses at one time.
Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.
This medicine can cause diarrhea, which may be a sign of a new infection. If you have diarrhea that is watery or bloody, call your doctor before using anti-diarrhea medicine.
Get emergency medical help if you have signs of an allergic reaction ro Nexium: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.
Call your doctor at once if you have:
severe stomach pain, diarrhea that is watery or bloody;
seizure (convulsions);
kidney problems – urinating more or less than usual, blood in your urine, swelling, rapid weight gain;
low magnesium – dizziness, fast or irregular heart rate, tremors (shaking) or jerking muscle movements, feeling jittery, muscle cramps, muscle spasms in your hands and feet, cough or choking feeling; or

new or worsening symptoms of lupus – joint pain, and a skin rash on your cheeks or arms that worsens in sunlight.
Taking esomeprazole long-term may cause you to develop stomach growths called fundic gland polyps. Talk with your doctor about this risk.
If you use esomeprazole for longer than 3 years, you could develop a vitamin B-12 deficiency. Talk to your doctor about how to manage this condition if you develop it.
Common Nexium side effects may include:
headache, drowsiness;
mild diarrhea;
nausea, stomach pain, gas, constipation; or
dry mouth.

Tell your doctor about all your current medicines. Many drugs can affect esomeprazole, especially:
cilostazol;
clopidogrel;
diazepam;
digoxin;
erlotinib;
iron-containing medicines (ferrous fumarate, ferrous gluconate, ferrous sulfate, and others);
methotrexate;
mycophenolate mofetil;
rifampin;
St. John’s wort;
tacrolimus;
warfarin (Coumadin, Jantoven);
antifungal medication – ketoconazole, voriconazole; or
HIV/AIDS medication – atazanavir, nelfinavir, saquinavir.
This list is not complete and many other drugs may interact with esomeprazole. This includes prescription and over-the-counter medicines, vitamins, and herbal products. Not all possible drug interactions are listed here.
