مقدمه: افسردگی مرضی یکی از اختلالات شایع روانی است که چارچوبهای شناختی انسان در شکلگیری و درمان آن مؤثر است. نگرش توحیدی به عنوان یک چارچوب شناختی محبوب مردم و منتشر عواطفی با خود به همراه میآورد که لازمست مورد بررسی قرار گیرد.
روشها: این بررسی روی آیات قرآن کریم و با استفاده از منابع و نرمافزارهای نوری مربوطه انجام گرفته و اطلاعات حاصله در غالب پیشگیری اولیه و ثانویه از افسردگی طبقهبندی و مورد بحث قرار گرفتهاند.
یافتهها: سه محور اساسی این نگرش در پیشگیری از افسردگی و درمان آن عبارتند از: ایجاد تفکر مثبت و روحیه پویا، رفع نگرشهای منفی، رفع علائم و تظاهرات افسردگی. اعطای بینش کلی در مورد هستی و پاسخگویی به سؤالات انسان، اعطای ایمان و حکمت، اعطای نگرش همراه با توکل و رضایت به قضای الهی، نحوه برخورد با فقر و نگرانیهای اقتصادی، نحوه برخورد با مشکلات خانوادگی و سایر مصائب زندگی از جمله محورهای مورد بحث در این مقاله تکیه بر آیات قران کریم میباشد.
نتیجهگیری: نگرش توحیدی با طرح محورهای پیشگفت در تأمین عواطف رضایتبخش و متعادل انسان نقش اساسی دارد و به عنوان یک چارچوب شناختی مؤثر در پیشگیری در سطوح اجتماعی و فردی، میتواند در برهریزیهای پیشگیری و درمان مورد استفاده قرار گیرد. چارچوبهای شناختی حاصل از این رویکرد میتواند مبنایی برای رواندرمانی مبتلایان به افسردگی مورد استفاده قرار گیرد. در مقاله به تشابه و تفاوت این نگرش با سایر چارچوبهای شناختی اشاره شده است.

مقدمه: افسردگي يکي از شايع ترين اختلالات روانشناختي در کودکان و نوجوانان است. از دهه قبل، همزمان با افزايش شيوع افسردگي در سنين پايين تر توجه زيادي به تشخيصو درمان افسردگي در کودکان و نوجوانان جلب شده است.هدف: اين پژوهش با هدف تعيين فراواني نشانه هاي افسردگي در کودکان مقطع راهنمايي و ارتباط آن با برخي عوامل فردي و خانوادگي مي باشد.روش کار: اين پژوهش مقطعي از نوع توصيفي تحليلي و روش نمونه گيري از نوع تصادفي به صورت خوشه اي چند مرحله اي بود. به اين ترتيب که ابتدا نواحي 1 و 2 آموزش و پرورش شهر رشت به عنوان دو خوشه اصلي در نظر گرفته شد. در مرحله دوم در هر خوشه به طور جداگانه بر اساس جنسيت (مدارس دخترانه پسرانه)، نوع مدرسه (دولتي- غيردولتي) و پايه تحصيلي (ششم، هفتم و سوم راهنمايي)، جمعا 24 طبقه در دو ناحيه 1 و 2 محاسبه گرديد. تعداد کل نمونه 618 نفر بر اساس نتايج مطالعه مقدماتي تعيين گرديد. با توجه به اينکه کل هر کلاس خوشه در نظر گرفته شد، تعداد نمونه ها به 775 نفر (31 خوشه) افزايش يافت که در نهايت 753 نفر از والدين فرم تکميل شده را برگرداندند. ابزار تحقيق شامل پرسشه افسردگي کودکان و نوجوانان (CDI) Children Depression Inventory و پرسشه مشخصات دموگرافيک (فردي- خانوادگي) بود. پرسشه همراه با پاسخ ه و از طريق پرسشگران آموزش ديده در اختيار آزمودني ها قرار داده شد و از آنان خواسته شد آنچه را در همان لحظه احساس مي کنند با صداقت و دقت جوابدهند. پرسشه افسردگي کودکان و نوجوانان قبلا روانسجي شده و بهترين نقطه برش براي آن در ايران، 22 تعيين شده است. داده ها با استفاده از آمار توصيفي و آزمون هاي آمار استنباطي (آزمون هاي مجذور کاي، رگرسيون لجستيک و تي مستقل)، مورد تجزيه و تحليل آماري قرار گرفتند.نتايج: بيشتر واحدهاي مورد پژوهش (%37.6) 13 سال سن داشتند. فراواني دخترها 398 نفر (%52.9) و پسرها 355 (%47.1) نفر بود. فراواني کودکان در کلاس هاي ششم،هفتم و سوم راهنمايي به ترتيب 268، 249 و 236 نفر بود. 7.8 درصد از کودکان در مدارس دولتي و 29.2 درصد در مدارس غيردولتي مشغول به تحصيل بودند. اکثريت واحدهاي مورد پژوهش داراي پيشرفت تحصيلي مثبت (%64.1) و فاقد پدر و مادر با سابقه افسردگي (به ترتيب %87 و %87.9) بودند. نتايج بيانگر فراواني نشانه هاي افسردگي در ده درصد نمونه ها بود. وجود نشانه هاي افسردگي در پسران در 8.5 درصد و در دختران در 11.3 درصد ، بر حسب پايه هاي کلاسي در پايه هاي ششم، هفتم، سوم راهنمايي به ترتيب 11.6، 5.2 و 13.1 درصد، در مدارس دولتي و غيردولتي به ترتيب 12.2 و 4.5 درصد، بر حسب داشتن و نداشتن پيشرفت تحصيلي به ترتيب 1.6 و 8.9 درصد، بر حسب سابقه افسردگي پدر و مادر به ترتيب 2 و 7.7 درصد و برحسب وجود بيماري هاي مزمن در پدر و مادر به ترتيب 7.9 و 1.5 درصد بوده است. از نظر درآمد ماهيانه، خانواده هاي با درآمد ماهيانه کمتر از پانصد هزار تومان، بيشترين (%14.4) ميزان افسردگي را داشته اند. بيشترين ميزان نشانه هاي افسردگي در کودکان از نظر تحصيلات پدر و مادر به ترتيب با سطح تحصيلي بي سواد و ديپلم (%12.3 و %4.8) بوده است. بيشترين ميزان نشانه هاي افسردگي در کودکان از نظر شغل پدر و مادر به ترتيب با مشاغل بيکار و خانه دار 19 و 1.3 درصد بوده است. بيشترين ميزان نشانه هاي افسردگي (%1.9) در کودکان از نظر وضعيت سکونت، در خانواده هايي با مساکن استيجاري بوده و بيشترين ميزان افسردگي (%33.3) در کودکان از نظر ساختار خانواده، در خانواده هايي با فوت مادر بوده است.نتايج نشان داد نشانه هاي افسردگي با پايه کلاسي (P=.8)، نوع مدرسه (p=.1) و عدم سابقه افسردگي در پدر (p=.5)، از لحاظ آماري داراي ارتباط معني دار مي باشد. بطوريکه ميزان نشانه هاي افسردگي در پايه سوم راهنمايي بيشترين مقدار بوده است. دانش آموزان مدارس دولتي نشانگان افسردگي بيشتري نسبت به دانش آموزان مدارس غيردولتي داشتند (odds ratio:2.5، 1.3-5.1: CI%95) دانش آموزاني که داراي پدري با سابقه افسردگي بودند، از علايم و نشانه هاي افسردگي کمتري برخوردار بودند (%2 در برابر %11).نتيجه گيري: گرچه وجود نشانگان افسردگي بر اساس ابزار مورد استفاده در اين تحقيق لزوما مترادف با وجود اختلال افسردگي از نظر شدت و طول مدت علايم نمي باشد، امارابطه معنادار بين نمرات افسردگي و بعضي از عوامل فردي و خانوادگي اهميت در نظر گرفتن اين عوامل را در طراحي بره هاي پيشگيري کننده براي افسردگي کودکان نشان مي دهد.
