زمینه و هدف: زایمان زودرس 8% تا 10% حاملگی‌ها را شامل می‌شود. داروهای مختلفی جهت مهار زایمان زودرس و درمان بعد از آن به کار گرفته شده‌اند. هدف از مطالعه حاضر، مقایسه اثر شیاف واژینال پروژسترون و قرص ایزوکسوپرین خوراکی در افزایش فاز نهفته پس از کنترل زایمان زودرس است.
مواد و روش‌ها: این مطالعه به صورت کارآزمایی بالینی بر روی 100 زن باردار با علایم زایمان زودرس مراجعه‌کننده به بیمارستان افضلی‌پور کرمان، از تاریخ آذر ماه 1388 تا بهمن ماه 1389 انجام شد. زنان پس از کنترل انقباضات رحمی، به صورت تصادفی به دو گروه تقسیم شدند. به یک گروه شیاف واژینال پروژسترون 400 میلی‌گرم روزانه و به گروه دیگر قرص ایزوکسوپرین خوراکی 10 میلی‌گرم سه بار در روز تجویز شد. دو گروه تا زمان زایمان پی‌گیری و از نظر اثر‌بخشی و عوارض دارویی مقایسه شدند. داده‌ها با استفاده از آزمون‌های آماری مجذور کای و t مستقل تحلیل گردیدند.

یافته‌ها: دو گروه از لحاظ متوسط فاز نهفته تا زایمان، متوسط سن حاملگی زمان زایمان، عود زایمان زودرس، وزن زمان تولد نوزاد، بستری نوزادان در بخش مراقبت‌های ویژه نوزادان و زردی نوزادی یکسان بودند. در گروه پروژسترون هیچ عارضه دارویی مشاهده نشد اما در گروه ایزوکسوپرین 6/11% دچار عوارض دارویی شدند، که این اختلاف در دو گروه از لحاظ آماری معنی‌دار نبود.
نتیجه‌گیری: با توجه به نتایج این مطالعه، به نظر می‌رسد به علت عوارض مادری کمتر پروژسترون واژینال، این دارو بتواند گزینه مناسب‌تری در درمان نگهدارنده پس از توقف زایمان زودرس باشد.

من در هفته ی ششم بارداری دچار لکه بینی شدم و پزشک قرص Utrogestan 200 را برایم تجویز کردند.روزی یک عدد.از زمانی که این قرص را میخورم حدود یکساعت بعد از آن دچار سرگیجه ی شدید میشوم.به تازگی اطراف ران هایم نیز قرمز شده و مانند گرمک جوش های ریز قرمز زده است. آیا از عوارض قرص است؟ باید مصرف آن را ادامه بدهم یا اینکه دارویی جایگزین مصرف کنم؟البته لکه بینی ام کاملا قطع شده است. و فقط سه عدد از این قرص را تا به امروز مصرف نموده ام.

Everything you need to know about taking Utrogestan
Utrogestan capsules are used as part of hormone replacement therapy (HRT) for the menopause in women who still have their womb. They are prescribed to be taken alongside oestrogen-based HRT to oppose the effect of the oestrogen on the womb lining.
Taking progesterone with oestrogen is known as combined HRT.
In women who still have their womb (ie if you’ve not had a hysterectomy), oestrogen-based HRT can stimulate the growth and build-up of the womb lining (endometrium). This can lead to a slightly increased risk of developing endometrial cancer if the growth is unopposed.
The progesterone in Utrogestan capsules works by opposing oestrogen’s effect on the womb lining. It causes the womb lining to be shed, like a period, which reduces the risk of developing endometrial cancer. Utrogestan capsules are taken as part of each monthly oestrogen cycle.

Combined HRT using an oestrogen plus Utrogestan capsules may not be suitable for some women. Make sure your doctor knows if:
Utrogestan tablets can be taken in one of two ways each month, as described below. Follow the instructions given by your doctor for which method you should use.
Utrogestan capsules should be swallowed whole with a drink of water. They should be taken without food on an empty stomach.
If you forget to take a dose take it as soon as you remember, unless it’s nearly time for your next dose. In this case just leave out the missed dose and take your next dose as usual when it is due. Do not take a double dose to make up for a missed dose.
Medicines and their possible side effects can affect people in different ways. The following are some of the side effects that may be associated with Utrogestan capsules. Just because a side effect is stated here does not mean that all people taking this medicine will experience that or any side effect.
Common side effects (affect between 1 in 10 and 1 in 100 people)
Uncommon side effects (affect between 1 in 100 and 1 in 1000 people)
Very rare side effects (affect fewer than 1 in 10,000 people)
Taking combined HRT has been shown to slightly increase the risk of getting breast cancer and blood clots (eg deep vein thrombosis/pulmonary embolism), although the overall risk is still small. You can read more about these risks here:
Women on HRT should have regular breast examinations and mammograms. Examine your own breasts regularly and see your doctor if you notice any changes in your breasts.
Stop taking your HRT and see your doctor immediately if you get any of the following, as they could be symptoms of a blood clot: stabbing pains or swelling in one leg; pain on breathing or coughing; coughing up blood; unexplained breathlessness; sudden chest pain.
The risk of blood clots forming in the veins (thromboembolism) while taking HRT may be temporarily increased if you experience major trauma, have surgery, or are immobile for prolonged periods of time (this includes travelling for over three hours). Your doctor may recommend that you stop taking HRT for a period of time (usually four to six weeks) prior to any planned surgery, particularly abdominal surgery or orthopaedic surgery on the lower limbs, or if you are to be immobile for long periods. The risk of blood clots during long journeys may be reduced by appropriate exercise during the journey and possibly by wearing compression stockings. Ask your doctor or pharmacist for advice.
