کواتیاپین (Quetiapine) با نام تجاری سروکوئل (SEROquel) یک داروی ضد جنون می باشد که با تغییر اعمال مواد شیمیایی در مغز کار می کند.

کواتیاپین برای درمان بیماران اسکیزوفرنی در کودکان و بزرگسالان که حداقل 13 سال سن دارند استفاده می شود.

کواتیاپین همچنین برای درمان اختلال دو قطبی (افسردگی شیدایی) در کودکان و بزرگسالان که حداقل 10 سال سن دارند مورد استفاده قرار می گیرد.

کواتیاپین ممکن است همراه با داروهای ضد افسردگی مورد استفاده در درمان اختلال افسردگی اساسی در بزرگسالان قرار گیرد.

قرص seroquel

کواتیاپین طولانی رهش به نام سروکوئل ایکس آر (Seroquel XR) برای استفاده تنها در بزرگسالان بالاتر از 18 سال تجویز می شود.

کواتیاپین برای درمان مشکلات روانی ناشی از زوال عقل مورد تایید نیست .

کواتیاپین برای استفاده در تمام کودکان مورد تایید نمی باشد٬ اگر پزشک شما برای برای کودک شما کواتیاپین تجویز کرده است، با دکتر صحبت کنید تا مطمئن شوید که کواتیاپین برای کودک شما مناسب است.

مطمئن شوید که شما با خیال آسوده می ​​توانید این دارو را استفاده نمایید، اگر شما دارای هر یک از شرایط زیر می باشید با پزشک خود در جریان بگذارید :

بیماری کبد و یا کلیوی.
بیماری های قلبی، اختلالات ریتم قلبی، سابقه حمله قلبی یا سکته مغزی.
فشار خون بالا و یا پایین.
سابقه کمبود سلول های سفید خون (WBC)، .
تست های تیروئید غیر طبیعی و یا سطح پرولاکتین.
تشنج یا صرع؛
آب مروارید.
کلسترول بالا و یا تری گلیسرید.
سابقه شخصی یا خانوادگی ابتلا به دیابت.
مشکل در بلع.

مصرف داروهای ضد جنون در 3 ماه آخر بارداری ممکن است مشکلات را در نوزاد بوجود آورد٬ قبل از مصرف این دارو در دوران بارداری با پزشک خود مشورت نمایید

اگر شما در حالی که کواتیاپین مصرف می کنید باردار شدید ، از متوقف آن بدون مشورت دکتر خودداری نمایید ٬ از جمله نشانه های ترک دارو ، اختلالات تنفسی ، مشکلات تغذیه ، ایراد گیری کردن ، لرزش ، و عضلات شل یا سفت می باشد. با این حال، شما ممکن است بعد از ترک دارو مشکلات دیگری در دوران بارداری برای شما بوجود آید٬ به همین خاطر در صورت بارداری هرگز این دارو را بدون مشورت پزشک خود قطع نکنید .

کواتیاپین می تواند به شیر مادر منتقل شود و به کودک شیرخوار آسیب برساند. شما نباید در دوران شیردهی از این دارو استفاده نمایید .

اگر شما پس از مصرف این دارو دچار هر یک از عارضه های جدی زیر شدید٬ با پزشک خود تماس گرفته و او را در جریان قرار دهید .

احساس سبکی سر ،
عضلات بسیار سفت ( سفت و سخت ) ، تب بالا، عرق کردن ، گیجی، ضربان قلب سریع یا ناهموار، لرزش .
کشش و یا غیر قابل کنترل حرکات چشم، لبها، زبان ، صورت، بازوها و یا پاها .
بلع مشکل ،
تورم پستان یا ترشحات ( در مردان و زنان ) ، از دست رفته دوره قاعدگی ، ناتوانی جنسی ، کاهش میل جنسی .
تاری دید، تونل دید ، درد چشم، یا دیدن هاله در اطراف چراغ .
ضعف ناگهانی و یا احساس ناخوشی ، تب، لرز، علائم سرماخوردگی و آنفلوآنزا ، سرفه ، گلو درد، لثه قرمز و متورم ، زخم های دردناک دهان ، زخم های پوست، تنفس مشکل

سرگیجه، خواب آلودگی ، از دست دادن انرژی، احساس خستگی .
افزایش اشتها، افزایش وزن .
خشکی دهان .
تهوع، استفراغ ، درد معده یا ناراحتی ، یبوست .

قبل از مصرف کواتیاپین با قرص خواب، داروهای درد مخدر، relaxer عضلانی، و یا دارو برای اضطراب، افسردگی، یا تشنج با پزشک خود مشورت نمایید .
بسیاری از داروها می تواند با کواتیاپین تداخل ایجاد کنند. همه فعل و انفعالات که اینجا ذکر شده است امکان پذیر نمی باشند٬ در هر صورت در مورد تمام داروهایی که استفاده می نمایید با پزشک خود در جریان بگذارید ٬ از جمله داروهای زیر :

lumefantrine، متوکلوپرامید، mifepristone.
داروهای ضد سرطان – تری اکسید آرسنیک، bexarotene، nilotinib، toremifene، vandetanib، vemurafenib. داروهای ریتم قلب – آمیودارون، disopyramide، dofetilide، درونیدرونات، flecainide، پروکائین آمید، quinidine، sotalol. داروهای HIV / AIDS – ریتوناویر، ساکوییناویر. دارو برای درمان بیماری های روانی – iloperidone، پیموزاید، thioridazine، زیپرازیدون.

سلام اقای دکتر بنده ۳۸ سالمه از ساله ۹۱ تا الان شبی ۱ عدد کوتیاپین ۱۰۰ میخورم.ساعت خوابم کمتر شده و دچاره تشنجها و خفگی های مداوم در هر شب میشم.۳ تا قرص دیگه هم میخوردم که خودسرانه اونها رو در طی این چند سال حذف کردم.(سیتالوپرام.پرفنازین.دپاکین)آیا راهی هست که بتونم این قرص رو هم اصلا نخورم؟ یا دوزش رو کم کنم؟ یا اینکه بخوام یکباره این قرص رو نخورم مشکلی پیش میاد؟

ترانکوپین برای اختلال خواب هم تجویز میشه ولی باید نوع اختلال خواب و به عبارتی دلیلش مشخص بشه و با توجه به اون قرص تجویز بشه .
بهر حال توصیه میکنم در جهت پیدا کردن علت بیخوابی به پزشک مناسب مراجعه کنید مثل یک روانشناس . نمیشه تا آخر عمر برای بیخوابی دارو مصرف کرد . هیچ دارویی بدون عوارض نیست .

با سلام و وقت بخیر..
بنده شبها خوابم نمیبرد و از یه بیماری به نام ثبات خلقی که ظاهراً طیفی از اختلال دوقطبی هست رنج میبردم…
دکتربنده.. یه تا قرص دپاکین، کویتایپین، و قرص استرس پروپرانول بهم داد که هر شب بخورم…
هر جایی که سوال میکردم به هیچ وجه بهم توصیه نکردند که قرص دپاکین ر مصرف کنم.. به خاطر عوارض بسیار بدش…برای همین میخوام بدونم که اون دو قرص رو بدون قرص اولی آیا مصرف بشه مشکلی به ووجود میاره؟؟

موضوع عالیه سوالی که داشتم این است که من دپاکین رسپیریدون و فلوواکسامین شبا مصرف میکنم و ظهرها نصف کوتیاپین اما این دارو به شدت باعث خواب الودگی من میشه در طول روز وکارهامو مختل کرده ایا میتونم کوتیاپین رو با یکی از داروهایی که شب مصرف میکنم جا به جا کنم یعنی شب بخورم بسیار ممنون و سپاسگذار

بله شما ميتونيد اون رو همراه قرص هاي شبتون ميل كنيد و البته درس و علميش اين است كه همراه بااون داروها مصرف بشه چون كه اين دارو ثابت نگهداره موده اعصاب است و همزمان مصرف بشه بهتره و لازم به ذكر هست كه با مشورت پزشك ميتونيد يك چهارم در شب مصرف كنيد چون بسيار خواب اوره

مريض تنها مجاز به دريافت اطلاعات دارويي از داروساز يا پزشك معالج خود است،
منظور از مشورت با ارايه دهنده ي مراقبت هاي بهداشتي شخصي بجز اين دو فرد است؟

Quetiapin ist ein Arzneistoff, der zur Behandlung psychischer Störungen eingesetzt wird. Der Stoff aus der Gruppe der atypischen Neuroleptika ist angezeigt zur Behandlung von Schizophrenie sowie von manischen und depressiven Episoden, die bei einer bipolaren Erkrankung auftreten.

Quetiapin ist eine weiße, kristalline, wenig wasserlösliche Substanz. Das Dibenzothiazepin-Derivat ist strukturell Clozapin, Olanzapin und Zotepin ähnlich. Pharmazeutisch verwendet wird das wesentlich besser wasserlösliche Salz der Fumarsäure, das Fumarat.[1]

Quetiapin entfaltet seine Wirkung im Nervensystem, wo es biochemische Signalprozesse beeinflusst.

Quetiapin blockiert als Antagonist mehrere Rezeptoren, die mit seiner atypischen, antipsychotischen Wirksamkeit in Verbindung gebracht werden, insbesondere Serotonin-5-HT2-Rezeptoren und Dopamin-D2-Rezeptoren.[7] Durch die Blockade der ersteren wird Dopamin in erhöhter Menge freigesetzt, durch die Blockade der Dopamin-D2-Rezeptoren deren hemmender Effekt auf die Nervenzellen verhindert.

Durch den Agonisten Serotonin an 5-HT2A-Rezeptoren wird die Dopaminfreisetzung gehemmt; negative affektive und kognitive Symptome der Schizophrenie gelten als Hinweis auf einen Dopaminmangel im präfrontalen Kortex. Gleichzeitig soll Serotonin am 5-HT2A-Rezeptor im dorsolateralen präfrontalen Kortex mitverantwortlich für Depressionen und Ängste sein.

Durch den 5-HT2A-Rezeptor-Antagonismus von Quetiapin wird also Dopamin freigesetzt; im präfrontalen Kortex befinden sich vergleichsweise wenige D2-Rezeptoren und viele D1-Rezeptoren, welche dadurch angeregt werden. Im limbischen System hingegen soll die Serotonin-2A-Regulation bei der Dopaminfreisetzung nur eine untergeordnete Rolle spielen, so dass in dieser Gehirnregion nur eine geringfügig erhöhte D2-Rezeptorbesetzung durch freigesetztes Dopamin vorliegt, welche jedoch durch den D2-antagonistischen Effekt von Quetiapin kompensiert wird.

Die Blockade der D2-Rezeptoren in nigrostriatalen Strukturen ist so genanntes loose binding („lockere Bindung“), ähnlich dem Clozapin, d. h., der Komplex Antipsychotikum-D2low (niedrigaffiner Dopamin-2-Rezeptor) wird leicht durch den physiologischen Liganden (Dopamin) gelöst, wodurch für Quetiapin (wie auch Clozapin) ein sehr niedriges EPMS-induzierendes Potenzial postuliert wird; Quetiapin scheint nach Clozapin das zweitniedrigste EPMS-Potenzial von allen Atypika zu haben.[8] Außerdem hat Quetiapin eine σ-Rezeptor-antagonistische Wirkung, deren Bedeutung jedoch noch nicht geklärt ist.

Der aktive Metabolit Norquetiapin (N-Desalkylquetiapin) hemmt durch eine Bindung am Noradrenalin-Transporter (NAT) die Wiederaufnahme von Noradrenalin,[9] er zeigt ferner schwache partialagonistische Effekte an 5-HT1A-Rezeptoren und antagonistische Effekte an 5-HT2C-Rezeptoren. Die agonistischen Effekte an 5-HT1A-Rezeptoren gelten als ein Grund für die verbesserten kognitiven und affektiven Fähigkeiten, da sie zur Dopaminfreisetzung im präfrontalen Kortex beitragen.

Da durch Quetiapin Histamin-H1-Rezeptoren bereits in niedriger Dosierung blockiert werden, können vorübergehende Somnolenz tagsüber und eine Besserung des gestörten Tag-Nacht-Rhythmus auftreten.

Quetiapin wird in der Leber überwiegend über das Cytochrom-P-450 3A4 (CYP3A4), das hauptsächlich für den Metabolismus von Quetiapin verantwortlich ist, abgebaut.[10][11] Dabei entsteht der aktive Metabolit N-Desalkylquetiapin (Norquetiapin),[12][13] der eine Halbwertszeit von etwa zwölf Stunden besitzt. N-Desalkylquetiapin hat eine mäßige bis hohe Affinität zu mehreren Muskarin-Rezeptor-Subtypen.

Die Pharmakokinetik von Quetiapin war nach gleichzeitiger Gabe der Antidepressiva Imipramin (das als CYP2D6-Hemmer bekannt ist) oder Fluoxetin (das als CYP3A4- und CYP2D6-Hemmer bekannt ist) nicht signifikant verändert. Gleiches gilt für die Gabe der Antipsychotika Risperidon oder Haloperidol mit Quetiapin.

Die gleichzeitige Anwendung von Quetiapin und Thioridazin hingegen führte zu einem Anstieg der Quetiapin-Clearance um ca. 70 %. Dies führt zu Nebenwirkungen, die auf anticholinergen Wirkungen beruhen. Quetiapin sollte daher bei Patienten, die Arzneimittel mit anticholinergen (muskarinischen) Wirkungen erhalten, mit Vorsicht angewendet werden.

Quetiapin selbst hat eine Halbwertszeit von rund sieben Stunden.[11] Dies wird auch durch Daten einer PET-Untersuchung bestätigt, in der nachgewiesen wurde, dass Quetiapin an 5HT2- und D2-Rezeptoren bis zu zwölf Stunden bindet.[10] Hemmstoffe dieses Enzymsystems wie z. B. Ciprofloxacin, Erythromycin, Ketoconazol, Cimetidin und Grapefruitsaft können den Abbau von Quetiapin zum Teil deutlich verlangsamen.

Häufigste Nebenwirkungen sind Benommenheit, Schwindel, Schläfrigkeit, Kopfschmerzen und Gewichtszunahme insbesondere in der ersten Behandlungswoche. Auch durch orthostatische Dysregulation[11] bedingte Flüssigkeitsansammlungen im Gewebe (Ödem), erhöhter Puls, Blutdruckabfall (Hypotonie) vor allem beim Aufstehen und Stehen, Verdauungsstörungen und Verstopfung (Obstipation), Mundtrockenheit (Xerostomie) sowie eine reversible Verringerung der Anzahl weißer Blutkörperchen (Leukopenie) und eine veränderte Leberfunktion (Leberenzymanstieg ALT, AST) wurden häufig beobachtet. Gelegentlich tritt unter Quetiapin ein Restless-Legs-Syndrom auf.[14] Selten tritt ein malignes neuroleptisches Syndrom, sehr selten eine Leberentzündung oder schwerwiegende Hauterkrankungen mit Fieber und Blasenbildung an den Schleimhäuten (Stevens-Johnson-Syndrom) auf. Teilweise wird auch eine Verlängerung des QTc-Intervalls und bedingt dadurch ein erhöhtes Risiko für kardiovaskuläre Rhythmusstörungen beobachtet.[11]

Neben den genannten Anwendungsgebieten (Indikationen) wird Quetiapin in der Schweiz in Kombination mit Antidepressiva auch zur Steigerung der Wirksamkeit (Augmentation) in der Behandlung von therapieresistenten Zwangserkrankungen verwendet.[15]

Auch zur Vorbeugung gegen Rückfälle bei einer bipolaren Erkrankung wird Quetiapin eingesetzt.[16] Von dieser Anwendung wird jedoch in der S3-Behandlungsleitlinie für Bipolare Störungen abgeraten.[17]

Die Wirkung gegen depressive Phasen der bipolaren Erkrankung wurde in einer US-Studie belegt.[18] Die Zulassung für Quetiapin – als bislang einzigem Atypikum – wurde 2006 durch die FDA auf die Monotherapie solcher depressiven Phasen erweitert.[19] Im deutschen Sprachraum ist Quetiapin in dieser Indikation im November 2008 zugelassen worden.[20]

Auch bei unipolaren Depressionen bzw. Major Depression wird Quetiapin in retardierter Form seit einiger Zeit als Zusatztherapie bei Patienten, bei denen eine Monotherapie mit einem Antidepressivum nicht oder nur unzureichend wirkt, eingesetzt. Dies wurde im Oktober 2010 durch das Bundesinstitut für Arzneimittel und Medizinprodukte anerkannt und ist somit kein Off-Label-Use mehr, sondern offiziell zugelassen.[21]

Studienergebnisse lassen vermuten, dass Quetiapin bei schweren Formen der posttraumatischen Belastungsstörung genutzt werden kann.[22]

Bei der chemischen Synthese wird zunächst aus dem Lactam mit Phosphoroxychlorid das 11-Chlordibenzothiazepin synthetisiert. Durch nucleophile Substitution wird anschließend die Seitenkette eingeführt.[23]

Seroquel (quetiapine) is an antipsychotic medicine. It works by changing the actions of chemicals in the brain.

Seroquel is used to treat schizophrenia in adults and children who are at least 13 years old.

Seroquel is used to treat bipolar disorder (manic depression) in adults and children who are at least 10 years old.

Seroquel is also used together with antidepressant medications to treat major depressive disorder in adults.

Seroquel may also be used for purposes not listed in this medication guide.

Never take Seroquel in larger amounts, or for longer than recommended by your doctor. High doses or long-term use can cause a serious movement disorder that may not be reversible. Symptoms of this disorder include tremors or other uncontrollable muscle movements.

Stop using Seroquel and call your doctor at once if you have the following symptoms: very stiff (rigid) muscles, high fever, sweating, confusion, fast or uneven heartbeats, tremors, uncontrolled muscle movements, feeling light-headed, blurred vision, eye pain, increased thirst and urination, excessive hunger, fruity breath odor, weakness, nausea and vomiting.

Some young people have thoughts about suicide when first taking Seroquel. Stay alert to changes in your mood or symptoms. Report any new or worsening symptoms to your doctor.

Seroquel is not FDA approved for use in psychotic conditions related to dementia. Quetiapine may increase the risk of death in older adults with dementia-related conditions.

You should not use Seroquel if you are allergic to quetiapine.

To make sure Seroquel is safe for you, tell your doctor if you have:

liver or kidney disease;

heart disease, heart rhythm problems, a history of heart attack or stroke;

high or low blood pressure;

a history of low white blood cell (WBC) counts;

abnormal thyroid tests or prolactin levels;

seizures or epilepsy;

cataracts;

high cholesterol or triglycerides;

a personal or family history of diabetes; or

trouble swallowing.

Some young people have thoughts about suicide when first taking Seroquel. Your doctor will need to check your progress at regular visits. Your family or other caregivers should also be alert to changes in your mood or symptoms.

Taking antipsychotic medication during the last 3 months of pregnancy may cause problems in the newborn, such as withdrawal symptoms, breathing problems, feeding problems, fussiness, tremors, and limp or stiff muscles. However, you may have withdrawal symptoms or other problems if you stop taking your medicine during pregnancy. If you become pregnant while taking Seroquel, do not stop taking it without your doctor’s advice.

Seroquel can pass into breast milk and may harm a nursing baby. You should not breast-feed while you are using Seroquel unless directed by a doctor.

Do not give Seroquel to a child without a doctor’s advice. Extended-release Seroquel XR is for use only in adults and should not be given to anyone younger than 18 years old.

Never take Seroquel in larger amounts, or for longer than recommended by your doctor. Follow all directions on your prescription label. High doses or long-term use of quetiapine can cause a serious movement disorder that may not be reversible. Symptoms of this disorder include tremors or other uncontrollable muscle movements.

Take this medicine with a full glass of water. You may take immediate-release Seroquel with or without food. It is suggested that Seroquel XR be administered without food or with a light meal (< 300 calories), preferably in the evening.

Do not crush, chew, or break an extended-release tablet. Swallow it whole.

Seroquel may cause you to have high blood sugar (hyperglycemia). If you are diabetic, check your blood sugar levels on a regular basis while you are taking Seroquel.

You should not stop using Seroquel suddenly. Stopping suddenly may make your condition worse.

Blood pressure may need to be checked often in a child or teenager taking Seroquel.

Quetiapine can cause you to have a false positive drug screening test. If you provide a urine sample for drug screening, tell the laboratory staff that you are taking Seroquel.

Store at room temperature away from moisture and heat.

Take the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.

Seek emergency medical attention or call the Poison Help line at 1-800-222-1222. An overdose of quetiapine can be fatal.

Avoid drinking alcohol. Dangerous side effects could occur.

Seroquel may impair your thinking or reactions. Avoid driving or operating machinery until you know how this medicine will affect you.

Avoid getting up too fast from a sitting or lying position, or you may feel dizzy. Dizziness or severe drowsiness can cause falls, fractures, or other injuries.

Avoid becoming overheated or dehydrated during exercise and in hot weather. You may be more prone to heat stroke.

Get emergency medical help if you have signs of an allergic reaction to Seroquel: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.

Report any new or worsening symptoms to your doctor, such as: mood or behavior changes, anxiety, panic attacks, trouble sleeping, or if you feel impulsive, irritable, agitated, hostile, aggressive, restless, hyperactive (mentally or physically), more depressed, or have thoughts about suicide or hurting yourself.

Call your doctor at once if you have:

twitching or uncontrollable movements of your eyes, lips, tongue, face, arms, or legs;

mask-like appearance of the face, trouble swallowing, problems with speech;

a light-headed feeling, like you might pass out;

blurred vision, tunnel vision, eye pain, or seeing halos around lights;

severe nervous system reaction – very stiff (rigid) muscles, high fever, sweating, confusion, fast or uneven heartbeats, tremors, fainting;

high blood sugar – increased thirst, increased urination, hunger, dry mouth, fruity breath odor, drowsiness, dry skin, blurred vision, weight loss; or

low blood cell counts – sudden weakness or ill feeling, fever, chills, cold or flu symptoms, cough, sore throat, red or swollen gums, painful mouth sores, skin sores, trouble breathing.

Common Seroquel side effects may include:

trouble with movement;

dizziness, weakness;

increased appetite, weight gain;

sore throat;

dry mouth; or

nausea, vomiting, stomach pain, constipation.

This is not a complete list of side effects and others may occur. Call your doctor for medical advice about side effects. You may report side effects to FDA at 1-800-FDA-1088.

Seroquel can cause a serious heart problem if you use certain medicines at the same time, including certain antibiotics, antidepressants, heart rhythm medicine, antipsychotic medicines, and medicines to treat cancer, malaria, HIV or AIDS. Tell your doctor about all medicines you use, and those you start or stop using during your treatment with Seroquel.

Taking Seroquel with other drugs that make you sleepy or slow your breathing can cause dangerous or life-threatening side effects. Ask your doctor before taking a sleeping pill, narcotic pain medicine, prescription cough medicine, a muscle relaxer, or medicine for anxiety, depression, or seizures.

Many drugs can interact with quetiapine. Not all possible interactions are listed here. Tell your doctor about all your current medicines and any you start or stop using, especially:

antibiotic or antifungal medicines;

antiviral medicine to treat hepatitis or HIV/AIDS;

heart or blood pressure medicines;

medicine to treat mental illness;

St. John’s wort;

seizure medicine; or

medicines used to treat tuberculosis.

This list is not complete and many other drugs can interact with quetiapine. This includes prescription and over-the-counter medicines, vitamins, and herbal products. Give a list of all your medicines to any healthcare provider who treats you.

This leaflet answers some of the common questions people ask about SEROQUEL. It does not contain all the available information.

It does not take the place of talking to your doctor or pharmacist.

All medicines have risks and benefits. Your doctor will have weighed the risks of you taking it against the benefits they expect it will have for you.

SEROQUEL helps to correct chemical imbalances in the brain. It is used to treat conditions such as:

SEROQUEL belongs to a group of medicines called antipsychotics.

Ask your doctor if you have any questions about why it has been prescribed for you.
Your doctor may prescribe this medicine for another reason.

You may find it helpful to tell a friend or relative that you are suffering from these symptoms, and ask them to read this leaflet. You might ask them to tell you if they think your symptoms are getting worse, or if they are worried about any other changes in your behaviour.

It is not addictive.

This medicine is available only with a doctor’s prescription.

Do not take SEROQUEL if you have an allergy to

Some of the symptoms of an allergic reaction may include shortness of breath, wheezing or difficulty breathing; swelling of the face, lips, tongue or other parts of the body; rash, itching or hives on the skin or you may feel faint.

Do not give SEROQUEL to children or adolescents unless recommended by your doctor.
The effects of SEROQUEL have only been studied in children aged between 10 and 17 years with mania and in children aged between 13 and 17 years with schizophrenia. There is not enough information on its effects in children to recommend its use in other age groups or for other conditions.

SEROQUEL, as with other anti-psychotic medicines, is recommended for use with caution in the elderly and is not approved for treatment of dementia and behavioural disturbances. In clinical studies with this group of medicines for the treatment of dementia and behavioural disturbances in the elderly, have suggested an increased risk of cardiovascular adverse events including stroke, and increased mortality risk for elderly patients has been reported.

Do not use it after the use by (expiry) date printed on the pack.

Do not use it if the packaging is torn or shows signs of tampering.
If it has expired or is damaged, return it to your pharmacist for disposal.

If you are not sure whether you should start taking SEROQUEL, talk to your doctor or pharmacist.

Tell your doctor or mental health professional if you have any mental/mood changes or suicidal thoughts.

Depression and other mental illnesses can increase the risk of suicide. It is important to discuss all the risks of treating depression and mental illness as well as the risks of not treating it. You should discuss all treatment choices with your doctor, not just the use of antidepressants.

Patients (and caregivers of patients) need to monitor for any worsening of their condition and/or the emergence of thoughts of suicide or suicidal behaviour or thoughts of harming themselves and to seek medical advice immediately if these symptoms present.

Do not take SEROQUEL if you are pregnant or breastfeeding unless your doctor says so. Ask your doctor about the risks and benefits involved.

It is not known if it is safe for you to take it while you are pregnant. However, if you need to take it during your pregnancy, the doctor will discuss the risks and benefits of taking it with you.

It is recommended that you do not breastfeed while taking it, as it may pass into breast milk.

Before your doctor starts you on SEROQUEL your doctor may want to take some tests (eg blood tests, blood pressure, weight or height measurements). These tests may help to prevent side effects.

Tell your doctor if you are lactose intolerant.
SEROQUEL tablets contain lactose.

Tell your doctor or pharmacist if you are taking any other medicines, including any that you buy without a prescription from your pharmacy, supermarket or health food shop.

Some medicines and SEROQUEL may interfere with each other. These include:

These medicines may be affected by SEROQUEL, or may affect how well it works. You may need different amounts of your medicine, or you may need to take different medicines. Your doctor will advise you.

Your doctor and pharmacist have more information on medicines to be careful with or avoid while taking SEROQUEL.

If you have not told your doctor about any of these things, tell them before you take any SEROQUEL.

Follow all directions given to you by your doctor or pharmacist carefully.
They may differ from the information contained in this leaflet.

If you do not understand the instructions on the pack, ask your doctor or pharmacist for help.

Your doctor will tell you how many tablets you will need to take each day and how long you need to take it. This depends on your condition and whether or not you are taking any other medicines.

SEROQUEL is usually started as a low dose that will be gradually increased by your doctor. Your doctor will recommend a dose especially for you. This will depend on various factors including your age, condition being treated, other medical conditions (eg liver problems), other medicines you are taking and how you may react to SEROQUEL. Your doctor will monitor your condition and may change your dose depending on how you respond to it. Your doctor will use the lowest dose for the least amount of time to keep you well.

SEROQUEL is taken once or twice a day depending on your condition. Your doctor will tell you how you should take it.

Swallow your SEROQUEL tablets whole with a full glass of water.

You can take them with or without food.

Continue taking the tablets for as long as your doctor tells you.

SEROQUEL helps control your condition, but does not cure it. Therefore you must take it every day.

Do not stop taking it unless your doctor tells you to – even if you feel better.

If it is almost time for your next dose (within 6 hours), skip the dose you missed and take your next dose when you are meant to. Otherwise, take it as soon as you remember, and then go back to taking your medicine as you would normally.

Do not take a double dose to make up for the dose you have missed.

If you are not sure what to do, ask your doctor or pharmacist.

If you have trouble remembering when to take your medicine, ask your pharmacist for some hints.

Immediately telephone your doctor or the Poisons Information Centre (13 11 26) for advice or go to Accident and Emergency at your nearest hospital if you think that you or anyone else may have taken too much SEROQUEL. Do this even if there are no signs of discomfort or poisoning.
You may need urgent medical attention.

If you take too many SEROQUEL tablets you may feel drowsy, sleepy, dizzy or have fast heart beats.

Tell any other doctors, dentists, and pharmacists who are treating you that you are taking it.

If you are about to be started on any new medicines, tell your doctor, dentist or pharmacist that you are taking it.

Tell your doctor or mental health professional immediately, or go to the nearest hospital, if you have any of the following suicidal thoughts or other mental/mood changes:

Occasionally, the symptoms of depression may include thoughts of suicide or self-harm. These symptoms may continue or get worse during the early stages of treatment until the effect of the medicine becomes apparent. All mentions of suicide or violence must be taken seriously.

If you become pregnant while taking it, tell your doctor immediately.

If you plan to have surgery that needs a general anaesthetic, tell your doctor or dentist that you are taking it.

If you need to have any medical tests (including urine drug screening) while you are taking it, tell your doctor.
It may affect the results of some tests.

Be sure to keep all of your doctor’s appointments so that your progress can be checked.
Your doctor will check your progress and may want to take some tests (eg blood tests, blood pressure, weight or height measurements) from time to time. These tests may help to prevent side effects.

Do not stop taking it, or change the dosage, even if you are feeling better, without checking with your doctor.
If you stop taking it suddenly, your condition may worsen or your chance of getting an unwanted side effect may increase. To prevent this, your doctor may gradually reduce the amount of SEROQUEL you take each day before stopping completely.

Do not give it to anyone else, even if their symptoms seem similar or they have the same condition as you.

Do not take it to treat any other complaints unless your doctor tells you to.

Do not take any medicines that cause drowsiness while you are taking it, unless recommended by your doctor.

Be careful driving or operating machinery until you know how SEROQUEL affects you.
It can make some people dizzy or sleepy. Make sure you know how you react to it before you do anything that could be dangerous if you are dizzy or sleepy. Children should be careful when riding bicycles or climbing trees.

If it makes you feel light-headed, dizzy or faint, be careful when getting up from a sitting or lying position.
Standing up slowly, especially when you get up from bed or chairs, will help your body get used to the change in position and blood pressure. If this problem continues or gets worse, talk to your doctor.

Be careful when drinking alcohol while you are taking it.
Combining SEROQUEL and alcohol can make you more sleepy or dizzy. Your doctor may suggest you avoid alcohol while you are being treated with it.

Avoid getting over-heated or dehydrated – do not over-exercise, in hot weather stay inside in a cool place, stay out of the sun, do not wear too much or heavy clothing, drink plenty of water. Keep warm in cool weather.
SEROQUEL may affect the way your body reacts to temperature changes.

Avoid drinking large quantities of grapefruit juice.
This medicine may be affected by grapefruit juice. Talk to your doctor or pharmacist if you have any concerns.

Talk to your doctor or pharmacist about these things if you think they may bother you.

Tell your doctor or pharmacist as soon as possible if you do not feel well while you are taking it.

All medicines can have side effects. Sometimes they are serious, most of the time they are not. You may need medical treatment if you get some of the side effects. Some side effects may be related to the dose of SEROQUEL or may be more likely to occur during long term treatment. Therefore, it is important that you tell your doctor as soon as possible if you notice anything that is making you feel unwell, even if you think the problems are not connected with this medicine. Your doctor may then decide to adjust your dose or use a different medicine.

Ask your doctor or pharmacist to answer any questions you may have.

These side effects are usually mild. Some of these side effects may go away after a while.

Tell your doctor as soon as possible if you notice the following:

These are serious side effects. You may need medical attention.

If any of the following happen, tell your doctor immediately or go to Accident and Emergency at your nearest hospital:

These are very serious side effects. You may need urgent medical attention or hospitalisation.

Occasionally, SEROQUEL may be associated with changes in your liver function or blood (eg blood fat levels such as cholesterol or triglycerides, blood sugar levels, blood pressure, thyroid hormone levels, white blood cells). These can only be found when your doctor does tests from time to time to check your progress.

Other side effects not listed above may occur in some patients. Tell your doctor or pharmacist if you notice anything that is making you feel unwell.

Do not be alarmed by this list of possible side effects.
You may not experience any of them.

Keep your SEROQUEL tablets in the blister pack until it is time to take them.
If you take the tablets out of the blister pack they will not keep well.

Keep it in a cool dry place where the temperature stays below 30°C.

Do not store it or any other medicine in the bathroom or near a sink. Do not leave it in the car or on a window sill.
Heat and dampness can destroy some medicines.

Keep it where young children cannot reach it.
A locked cupboard at least one-and-a-half metres above the ground is a good place to store medicines.

If your doctor tells you to stop taking it or the tablets have passed their expiry date, ask your pharmacist what to do with any that are left over.

SEROQUEL tablets are film-coated. All tablets except 300 mg are embossed with “Q” and the strength on one side, and are plain on the other side. SEROQUEL 300 mg tablets are embossed with “Q” on one side and “300” on the other.

Tablet shape and colouring is as follows:

SEROQUEL 25 mg, 200 mg and 300 mg come in packs of 60 tablets. SEROQUEL 100 mg comes in packs of 90 tablets.

Each tablet contains quetiapine fumarate as the active ingredient equivalent to quetiapine 25 mg, 100 mg, 200 mg or 300 mg.

Other ingredients include:
