قاعدگی (پریود) در هر دختری و پس از رسیدن به سن بلوغ رخ می‌دهد و از نظر پزشکی یک فرد سالم هر ماه (هر ۲۸ روز یکبار) به مدت ۵ الی ۷ روز خونریزی دارد. مقدار خونریزی و طول مدت آن با توجه به ژنتیک، شرایط بدنی فرد و شرایط روحی آن شخص در طول ماه‌های مختلف ممکن است کمی تغییر کند. به طور کلی تغییر در میزان ترشح دو نوع هورمون استروژن و پروژسترون عامل اصلی بروز عادت ماهانه به شمار می‌آید. سلامت قاعدگی برای تمامی زنان بسیار اهمیت دارد و در صورت بروز کوچکترین اختلالی در آن فرد به پزشک خود مراجعه می‌کند و ممکن است برای برگشتن به روند طبیعی مجبور به استفاده از داروهایی مثل داروی نورتیسترون شوند. داروی نورتیسترون نوعی هورمون زنانه مصنوعی (پروژسترون) است که برای درمان اختلالات قاعدگی و موارد دیگر مورد استفاده قرار می‌گیرد. ما تلاش کرده‌ایم در این مطلب برای شما عزیزان اطلاعات مفید و کاملی از این دارو تهیه کنیم.

داروی نورتیسترون (Norethindrone) که امروزه با نام های تجاری و اختصاری Noristerat ،Noriday ،Micronor در بازار و داروخانه ها به چشم می خورد جزء دسته دارویی هورمون جنسی زنان قرار می گیرد که در درمان اختلالات قاعدگی، به تأخیر انداختن دوران عادت ماهیانه و داروی ضدبارداری به کار گرفته می شود. همانطور که در مقدمه نیز گفتیم این دارو که مثل هورمون طبیعی پروژسترون می باشد که کاربردهای مختلفی نیز دارد، گاهی در درمان بیماری های قاعدگی یا پریود زنان از آن استفاده می شود و در برخی مواقع به عنوان هورمون جانشین درمان (HRT) در درمان نوع مشخصی از سرطان پستان به کار می رود. ضمنا این دارو یا به تنهایی و یا به همراه یک نوع استروژن مورد استفاده قرار می گیرد. طبق نظر پزشکان و به طور میانگین میزان خطر مصرف بیش از دوز مجاز دارو کم و میزان وابستگی و اعتیاد به دارو نیز کم اعلام شده است و همچنین برای تهیه داروی نورتیسترون از داروخانه ها نیاز به نسخه پزشک است اما امروزه شاهد این هستیم که این دارو به صورت آزاد هم به فروش می رسد. 

ابتدا باید بدانید که داروی نورتیسترون به شکل های قرص، تزریقی و پماد پوستی در بازار موجود است اما نوع قرص این دارو از همه پرکاربردتر می باشد. بنابر تحقیقات انجام شده بهترین زمان مصرف دارو در نوع قرص آن (۱ تا ۳ بار در روز)، در نوع تزریق (هر هفته یک بار) و در نوع پماد پوستی (۲ بار در هفنه) اعلام شده است و اندازه دوز مجاز مصرفی دارو نیز به این شکل است که هر فرد باید طبق دستور پزشک از این دارو مصرف کند.

داروی نورتیسترون نیز مانند همه ی داروهای ضدبارداری عمل می کند و با افزایش میزان هورمون پروژسترون در بدن مانع از بروز قاعدگی در زنان در طول مدت مصرف می شود اما به محض قطع مصرف دارو بعد از ۲ تا ۳ روز قاعدگی رخ می دهد. اثرات این دارو ظرف چند ساعت پس از شروع درمان با دارو ظاهر می شوند و این اثرات به مدت ۲۴ ساعت پس از قطع مصرف هر دوز از دارو در بدن دوام خواهند داشت. به گزارش مجله پزشکی آسان طب برای قطع این دارو اگر در دوره کوتاه مدت مورد استفاده قرار بگیرد، می توان به محض رفع نیاز بدون هیچ مشکلی استفاده از  آن را قطع کنید اما اگر مدت زمان طولانی تری از آن استفاده شود برای قطع مصرف این دارو باید با پزشک مشورت کرد.قرص norethisterone چيست

از عوارض معمول مصرف این دارو می توان به تورم پاها و زانوها، چاقی، خونریزی، سردرد و افسردگی، اشاره کرد و همچنین مصرف این دارو به مدت طولانی باعث ایجاد یرقان می شود که این امر ناشی از نارسایی کبدی می گردد. اگر با هریک از این عوارض برخورد کردید سریعا با پزشک خود صحبت کنید.

√ اگر به بیماری میگرن مبتلا هستید و یا در گذشته دچار حملات صرع گشته اید.

√ اگر در گردش خون خود و یا قلبتان احساس ناراحتی می کنید.

√ اگر سنگ مثانه شدید دارید.

√ اگر از انواع داروهای دیگر استفاده می کنید.

√ مصرف دارو برای زنان حامله: اغلب مجاز نمی باشد زیرا احتمال دارد که موجب نقص در نوزاد شود.

√ مصرف دارو برای زنان شیرده: به دلیل اینکه دارو در شیر مادر تاثیر می گذارد و ممکن است سلامتی نوزاد را به خطر اندازد حتما با پزشک مشورت کنید.

√ مصرف دارو برای کودکان و اطفال: مجاز نیست.

√ مصرف دارو برای افراد بالای ۶۰ سال: اغلب مجاز نمی باشد و توصیه نمی شود، چون همانطور که در بالا گفتیم استفاده طولانی مدت از داروی نورتیسترون موجب آسیب های کبدی می شود لازم است در طول درمان حتما آزمایش های مربوط به کبد انجام شود.

√این دارو باید در یک محفظه سربسته در جای خشک و خنک دور از دسترس کودکان نگهداری شود.

Norethisterone acetate (NETA), also known as norethindrone acetate and sold under the brand name Primolut-Nor among others, is a progestin medication which is used in birth control pills, menopausal hormone therapy, and for the treatment of gynecological disorders.[1][2][3][4] The medication available in low-dose and high-dose formulations and is used alone or in combination with an estrogen.[3][4][5][6] It is taken by mouth.[5]

Side effects of NETA include menstrual irregularities, headaches, nausea, breast tenderness, mood changes, acne, increased hair growth, and others.[5] NETA is a progestin, or a synthetic progestogen, and hence is an agonist of the progesterone receptor, the biological target of progestogens like progesterone.[1] It has weak androgenic and estrogenic activity and no other important hormonal activity.[1][7] The medication is a prodrug of norethisterone in the body.[8][9]

NETA was patented in 1957 and was introduced for medical use in 1964.[10][11] It is sometimes referred to as a “first-generation” progestin.[12][13] NETA is marketed widely throughout the world.[4] It is available as a generic medication.[14]

NETA is used as a hormonal contraceptive in combination with estrogen, in the treatment of gynecological disorders such as abnormal uterine bleeding, and as a component of menopausal hormone therapy for the treatment of menopausal symptoms.[4]

NETA is available in the form of tablets for use by mouth both alone and in combination with estrogens including estradiol, estradiol valerate, and ethinylestradiol.[15][4] Transdermal patches providing a combination of 50 μg/day estradiol and 0.14 or 0.25 mg/day NETA are available under the brand names CombiPatch and Estalis.[15][4]

NETA was previously available for use by intramuscular injection in the form of ampoules containing 20 mg NETA, 5 mg estradiol benzoate, 8 mg estradiol valerate, and 180 mg testosterone enanthate in oil solution under the brand name Ablacton to suppress lactation in postpartum women.[16][17][18][19]

Side effects of NETA include menstrual irregularities, headaches, nausea, breast tenderness, mood changes, acne, increased hair growth, and others.[5]

NETA is a prodrug of norethisterone in the body.[8] Upon oral ingestion, it is rapidly converted into norethisterone by esterases during intestinal and first-pass hepatic metabolism.[9] Hence, as a prodrug of norethisterone, NETA has essentially the same effects, acting as a potent progestogen with additional weak androgenic and estrogenic activity (the latter via its metabolite ethinylestradiol).[1][7]

In terms of dosage equivalence, norethisterone and NETA are typically used at respective dosages of 0.35 mg/day and 0.6 mg/day as progestogen-only contraceptives, and at respective dosages of 0.5–1 mg/day and 1–1.5 mg/day in combination with ethinylestradiol in combined oral contraceptives.[7] Conversely, the two drugs have been used at about the same dosages in menopausal hormone therapy for the treatment of menopausal symptoms.[7] NETA is of about 12% higher molecular weight than norethisterone due to the presence of its C17β acetate ester.[2]

NETA, also known as norethinyltestosterone acetate, as well as 17α-ethynyl-19-nortestosterone 17β-acetate or 17α-ethynylestra-4-en-17β-ol-3-one 17β-acetate, is a progestin, or synthetic progestogen, of the 19-nortestosterone group, and a synthetic estrane steroid.[2][3] It is the C17β acetate ester of norethisterone.[2][3] NETA is a derivative of testosterone with an ethynyl group at the C17α position, the methyl group at the C19 position removed, and an acetate ester attached at the C17β position.[2][3] In addition to testosterone, it is a combined derivative of nandrolone (19-nortestosterone) and ethisterone (17α-ethynyltestosterone).[2][3]

Chemical syntheses of NETA have been published.[21]

Schering AG filed for a patent for NETA in June 1957, and the patent was issued in December 1960.[10] The drug was first marketed, by Parke-Davis as Norlestrin in the United States, in March 1964.[10][11] This was a combination formulation of 2.5 mg NETA and 50 μg ethinylestradiol and was indicated as an oral contraceptive.[10][11] Other early brand names of NETA used in oral contraceptives included Minovlar and Anovlar.[10]

Norethisterone acetate is the INN, BANM, and JAN of NETA while norethindrone acetate is its USAN and USP.[2][3][4]

NETA is marketed under a variety of brand names throughout the world including Primolut-Nor (major), Aygestin (US), Gestakadin, Milligynon, Monogest, Norlutate (US, CA), Primolut N, SH-420 (UK), Sovel, and Styptin among others.[2][3][4]

NETA is marketed in high-dose 5 mg oral tablets in the United States under the brand names Aygestin and Norlutate for the treatment of gynecological disorders.[6] In addition, it is available under a large number of brand names at much lower dosages (0.1 to 1 mg) in combination with estrogens such as ethinylestradiol and estradiol as a combined oral contraceptive and for use in menopausal hormone therapy for the treatment of menopausal symptoms.[6]

NETA has been studied for use as a potential male hormonal contraceptive in combination with testosterone in men.[22]

Norethisterone is a synthetic version of progesterone, a hormone involved in regulating the menstrual cycle in women. It can also be used to treat certain medical conditions such as endometriosis, heavy periods or painful periods. Utovlan is a branded version of norethisterone.

 

Progesterone levels fluctuate throughout the course of the month. Towards the end of the cycle, progesterone levels drop, causing the lining of the womb to break down and it is expelled via bleeding, known as a period. Norethisterone works to keep levels of progesterone elevated, so that the lining of the womb is not broken down. This stops bleeding. 

   

For norethisterone to be effective, you should take one tablet three time a day, starting three days before your period is due. Keep taking the tablet for as long as you want to be period free. Your period will recommence 2-3 days after stopping norethisterone. 

 

Norethisterone can be taken with or without food.

 

You should aim to taking norethisterone three days before your period is due. The closer you take it to your expected period, the higher the chances of spotting.

 

If you miss a dose of norethisterone, take it as soon as you remember. If you remember when it is time for your next dose, do not take two tablets together. 

 

Norethisterone tablets can be taken for a maximum of 20 days, giving 17 days period free (as it needs to be taken three days before the onset of a period).

 

Your period will start around 2-3 days after stopping norethisterone. 

 

Most women find that norethisterone is effective at delaying their period. Some women may find that they experience occasional spotting (small spots of blood that are visible on underwear), or light breakthrough bleeds. Taking norethisterone closer to the time of your expected period can increase the chances of this happening. It is important to try and take norethisterone three days before your expected period to reduce the chances of this spotting.  

 

Norethisterone cannot be taken with the contraceptive pill in any circumstance.

If you are taking the progestogen only pill, also known as the mini pill, there is no need to take a period delay tablet, as it is highly unlikely that you are having periods.

If you are taking a combined contraceptive pill, you can avoid having a period by taking two packets back to back (missing out the tablet free days). You can take up to three packets back to back. This is common practice and works for 21-day monophasic pills such as Microgynon, Yasmin, Rigevidon and Cilest. If is important to seek advice from a healthcare professional before doing so.

If you are taking a 21-day phasic pill such as Logynon or Binovium, you should consult your GP or pharmacist. This is because each pill contains a different amount of hormones and need to be taken in the correct order to be effective. 

  

Norethisterone can prevent pregnancy at a different dose. The dose required to delay periods is different to the dose required to prevent pregnancy, and will not protect you from getting pregnant. You may wish to consider a combined oral contraceptive pill and miss out the tablet-free interval if you wish to benefit from contraception.

 

Norethisterone tablets are an effective way in delaying periods. You should always check with your doctor beforehand to see if it is suitable for you to take these tablets, as it can be dangerous for some women (e.g. women with a personal or family history of blood clots). They all contain the same active ingredient (norethisterone) and the only difference may be in the inactive ingredients (such as excipients that bind the tablet together). So, what do you do? Pack your bag with tampons and accept that you’re going to feel a bit rubbish?

Norethisterone is a synthetic hormone designed to stop your period before it begins by mimicking progesterone. By artificially keeping your hormone levels steady, your body won’t shed its lining and your period won’t start.

You’ll have to take one tablet three times a day for a maximum of thirty days, beginning three days before your period is expected to start.

Norethisterone definitely isn’t something new. But thanks to readily available information on the internet and word of mouth, more and more women are relying on it when they want to skip their period for a big event, holiday, or date.

It can be ordered online or prescribed by a GP.

Kate, 29, is someone who’s previously used period delay pills to avoid having her period while she was on holiday.

‘I’ve used the contraceptive pill – I take Cilest – and another one that I used to get from the doctors when I was about 14, which wasn’t a contraceptive pill but was used to specifically delay your period,’ Kate told metro.co.uk.

‘When I was 14 and 15 I used it if I was going on a summer holiday, as obviously as a young teenager was entirely freaked out by periods and wouldn’t even want to LOOK at a swimming pool if I had one.

‘Now that I’m older I’ll still do it for holidays, or big parties like NYE, but with the contraceptive pill instead.’

Kate explains that for her, it’s the idea of avoiding all the hassle that prompts her take pills to put off her period.

‘For travels it’s just a nightmare to have to find bathrooms if you’re on long bus journeys all the time – and even in my late twenties it’s still annoying to have your period if you’re planning on being in swimwear at any point.

‘For parties it’s generally a hormone thing combined with all the trips to the bathroom with the queues – I don’t want those to take away from the enjoyment.’

Her experiences with specific period delay pills and using the contraceptive pill back to back without a break have been positive. She spoke to a doctor beforehand and says she’d ‘definitely do it again’.

Hayley Smith, the founder of Flow Aid, chooses to take Norethisterone pills because she has ‘really heavy and painful periods’.

‘I didn’t want them to interrupt my holiday,’ she told us.

‘I was told [about Norethisterone] by a friend, and then discussed it with a doctor who recommended the chosen pill.

‘The pill has worked each time I have taken it.’

That being said, Hayley has experienced some after-effects from using period delay pills.

‘I always found that it took a month or two for my periods to get back to normal. They were either lighter than normal or the pain lasted longer.

‘This always put me off taking the pills so I only took them if I absolutely needed to.’

So what’s the deal with Norethisterone? Is it really safe to take a pill to delay your period whenever you need to?

Dr Helen Webberley, the dedicated GP for Oxford Online Pharmacy, told metro.co.uk: ‘Norethisterone is a synthetic copy of the female hormone, progesterone.

‘When levels are naturally high (as in pregnancy, or when waiting for the fertilised egg) the lining of the womb is maintained.

‘When levels drop (if implantation does not happen) then the lining of the womb comes away as a period.

‘Taking it as a pill keeps the lining of the womb in place, delaying your period until you stop taking it.

‘Delaying your period every now and again using progesterone tablets is quite safe and can be a real life-saver when periods are a nuisance.

‘However, if you do it too often, you may find that you get some unpredictable and erratic bleeding. If you are finding that you want to do it very often then it would be better to take a regular, low dose contraceptive such as the progesterone-only pill.’

Many women use taking a contraceptive pill back to back without the usual seven day break as their go-to method.

Tamsyn Martin, 25, says that she takes Microgynon 30 back to back an average of twice a year, usually if she’s going on holiday and her period falls on the dates she’ll be abroad.

‘My doctor explained that I can simply take a second dosage of 21 days,’ she says. ‘Once these have been taken my period will come naturally after that.

‘Over the years I have used it to put off my period for an extra few weeks and haven’t experienced any drastic side-effects.

‘At most I will get some light cramps, usually around the time my period was meant to be due and some discharge.قرص norethisterone چيست

‘I know my body enough to stop taking the pill if I think it will harm me.’

Dr Kate Guthrie, spokesperson for the Royal College of Obstetricians and Gynaecologists (RCOG), also suggests back-to-backing your usual contraceptive pill as a better option if you’re frequently delaying your periods.

She also notes that Norethisterone is not a form of contraception (as many of us probably assume), so you can definitely still get pregnant while using it – unlike when you’re using a regular contraceptive pill.

‘Norethisterone is not a form of contraception so women who are not planning a pregnancy will need to continue using other methods of contraception,’ Dr Guthrie told metro.co.uk.

She adds that the effectiveness of norethistrone is not entirely reliable, and can vary between women – meaning it’s probably not wise to rely on the pill for the first time when you’re headed on holiday.

Oh, and you could experience some not-so-great symptoms.

‘Some of the negative side effects include: bloating, weight gain, short-term acne, breast tenderness, nausea, headaches and disturbances in mood and sex drive,’ says Dr. Guthrie. All of which may be more annoying than your actual period.

So let’s recap. What’s the best option when you’re stuck with awful, life-disrupting periods and really don’t want to deal with having one on holiday, at a party, or during a big week at work?

Norethisterone is safe and can work well for some women. But there can be side-effects, so it may be worth trying it out during a not so important week first to check how it’ll affect you.

But if you find yourself frequently wanting to delay your periods, going on a regular contraceptive pill is a healthier option – and one that has the added bonus of providing you with protection from unwanted pregnancy.

With both, however, you’ll absolutely need to have a chat with your GP first. Don’t buy pills online without a prescription, don’t assume something that works for your friends will work for you, and make sure you get help from a health expert when you plan to put off your period. It’s just another important thing to add to your holiday to-do list, ‘kay?
