هنوز هم تولید یک قرص خواب عالی که بدون عوارض جانبی انسان را هرچه سریعتر و به مدت طولانی به خواب ببرد، یک رویا است. چنین قرص سحرآمیزی میتوانست بسیاری از مشکلات بالقوه را حل کند. میلیونها نفر امروزه خواب کافی ندارند. به عنوان مثال یافتههای یک نظرسنجی انجام گرفته توسط مرکز کنترل و پیشگیری بیماریها در امریکا نشان میدهد که ۳۵ درصد آمریکاییها کمتر از ۷ ساعت در شب میخوابند. در این مقاله با مادهای به ملاتونین آشنا شده و میخوانید که آیا این ماده میتواند به داشتن خواب راحت کمک کند.
خواب خوب تنها باعث بالا رفتن احساس تندرستی نمیشود. اتفاق نظر مجامع علمی بر این است که خواب خوب بدن را در برابر بیماریهای قلبی، دیابت و چاقی محافظت میکند. در آزمایشگاههای خواب در نمونهی خون شرکت کنندگانی که به مدت سه هفته چرخهی خواب معیوبی داشتند، آثاری از بیماری دیابت دیده شده بود. اگر به اهمیت خواب شک دارید، باید به این نکته توجه کنید که هم اکنون دانشمندان بر این باورند که چرخههای آسایش و هوشیاری از اصول مهم تنظیمکنندهی بدن همهی موجودات زندهی کرهی زمین هستند. حتی کوچکترین سلولها هم دارای کد ژنتیکی برای تنظیم ریتم شبانهروزی هستند.
مطمئنا داروهای خواب فراوانی در بازار وجود دارد که متاسفانه نه تنها آنچنان اثربخش نیستند بلکه میتوانند خطرناک و اعتیادآور هم باشند.
بسیاری ترجیح میدهند با مراجعه به داروخانه یک داروی جایگزین چند هزار تومانی تهیه کنند که در تبلیغات از آن به عنوان داروی طبیعی و کاملا مطمئن یاد میشود. این دارو ملاتونین دارد که در کشور امریکا، بر خلاف سایر مواد شیمیایی، به عنوان مکمل غذایی به فروش میرسد. البته برخی شواهد علمی هم از این ادعا پشتیبانی میکنند.
اما داستان ملاتونین هم مثل همهی داروهای خواب پیچیده است. با وجود اینکه میدانیم ملاتونین یک مادهی شیمیایی است که بدن ما به طور طبیعی در طول خواب آن را تولید میکند، اما نحوهی عملکرد و فوائد آن نیازمند یک بررسی جامع و دقیق است.
ملاتونین در اصل در دههی ۱۹۵۰ کشف شد. در آن زمان پژوهشگرانِ یک آزمایشگاه پوست، گمان میکردند این ماده نقش مهمی در رنگ پوست بازی میکند. آنها دوز بالایی از این مادهی شیمیایی را مصرف کردند به این امید که پوستشان روشنتر شود اما همگی خوابآلود شدند.
چیزی که دانشمندان به آن دست یافتند این بود که ملاتونین هورمونی است که بدن را از شبهنگام باخبر میسازد. این ماده در غدهی صنوبریشکلی که در اعماق مغز قرار دارد ساخته میشود.
به گفتهی دیک وارتمن (Dick Wurtman)، دانشمند علوم شناختی در دانشگاه MIT که به کشف کارکرد ملاتونین در انسانها کمک شایانی کرده است، «در مهرهداران کوچک مثل قورباغه، غدهی صنوبری در حقیقت چشم سومی است که نور تاریک را دریافت میکند.» دانشمندان در ابتدا فکر میکردند که این غده تنها در پستانداران کارکردی داشته است اما به تدریج به عضوی باقیمانده و بدون کارکرد بدل شده که در عرض چند میلیون سال به کلی ناپدید خواهد شد.»
اما در دههی ۱۹۶۰، وارتمن و تیمش در MIT دریافتند که نور هنوز هم از طریق چشم به غدهی صنوبری در پستانداران میرسد و ترشح ملاتونین را کنترل میکند که در طول روز به خاطر وجود نور مهار میشود. در شب تا وقتی که در معرض نور روشن یا نور آبی که ترشح ملاتونین را متوقف میکند و ما را بیدار نگه میدارد نباشیم، ملاتونین در جریان خون گردش دارد.
هنگامی که ملاتونین در جریان خون قرار میگیرد به عنوان یک پیامرسان عمل میکند. واکنش بدن به این پیامها به نوع حیوان بستگی دارد. برای مثال، موشها شبگرد هستند بنابراین ممکن است نقش ملاتونین بیدار نگه داشتن حیوان باشد. در انسانها و سایر موجوداتِ روزگرد، این ماده بدن را برای استراحت آماده میکند و کمک میکند که بدن در شب حالت سکون و آسایش را حفظ نماید.
به نظر میآید ملاتونین در انسانها به دو شیوه باعث خواب میشود:
۱. این باور وجود دارد که ملاتونین به طور وسیع، قسمتهایی از بدن که هنوز بیدار هستند را سرکوب میکند البته نحوهی عملکرد آن دقیقا مشخص نیست. همچنین ممکن است ملاتونین نقشی در پایین آوردن دمای بدن که لازمهی خواب است، داشته باشد.
۲. ملاتونین سرعت کلی ساعت بدن را تنظیم میکند. دانشمندان نشان دادهاند ساعت بدن افرادی که در بعدازظهر ملاتونین گرفتهاند فریب میخورد و فکر میکند دیروقت است و این افراد خیلی زود خوابآلوده میشوند. مصرف ملاتونین برای کسانی که دچار پرواززدگی شدهاند و بدنشان تصور میکند در یک منطقهی زمانیِ روزهنگام قرار دارند، میتواند سودمند باشد.
در بررسی و ارزیابی ملاتونین، سوالات بزرگی در ذهن پژوهشگران شکل گرفته است:
آیا ملاتونین به انسانهایی که از بیخوابی رنج میبرند کمک میکند که زودتر به خواب بروند؟
آیا ملاتونین به انسان کمک میکند که خواب کاملی داشته باشد؟
آیا ملاتونین به چرخهی خواب انسانی که وارد منطقهی زمانی جدیدی شده است کمک میکند؟
آیا ملاتونین برای افرادی که میخواهند خود را با شیفتهای جدید کاری وفق دهند سودمند است؟
و آیا ملاتونین مطمئنا ساعت بدن را تغییر میدهد؟
برای پاسخ به این سوالات مطالعات فراوانی بر روی جمعیتهای مختلف با مصرف دوزهای متفاوت انجام شده است که کار مقایسهی مستقیم این مطالعات را سخت کرده و دریافت ما از نتایج را به چالش کشانده است. اما فراتحلیلهایی که در مورد ملاتونین وجود دارد شواهدی را نشان میدهند که برای پاسخ به سوالات بالا میتواند مفید باشد.
پتی دویستر (Patty Deuster)، دانشمند و محقق علوم سلامت در دانشگاه یونیفورمد سرویس (Uniformed Services)، در سال ۲۰۱۴ نویسندهی ارشد یک مقالهی بررسی سیستماتیک مطالعات مربوط به ملاتونین بوده است. او و تیمش ۳۵ پژوهش تصادفی صورت گرفته توسط دارونما بر روی افراد سالمی را که بعضی از آنها دچار بیخوابی بودند گردآوری کردند. به گفتهی دویستر تاثیر ملاتونین بر روی بیخوابی مثبت اما ضعیف بوده است و به مطالعات با شرکتکنندگان بیشتری نیاز است.
همچنین یافتههای حاصل از مرور مطالعات توسط دویستر، تاثیر مثبت ضعیفی را بر روی شروع خواب، استمرار خواب و پرواززدگی نشان داده است. شواهد در خصوص کارکنان شیفتی و تغییر ساعت بدن بینتیجه بوده است. با این حال بعضی از آنها اعتقاد دارند ملاتونین را باید در ساعات پایانی بعدازظهر مصرف کرد زیرا در این زمان احتمال تغییر ساعت بیولوژیکی بسیار بالاتر است. گزارش دویستر تنها به مطالعات تغییر فاز ساعت بدن با مصرف ملاتونین در شب معطوف شده است.
سازمانهای بینالمللی سلامت نیز مطالعات صورت گرفته بر روی ملاتونین را تحلیل کردهاند.
در اروپا، در برخی کشورها ملاتونین تنها با ارائهی نسخهی پزشکی به فروش میرسد. در سال ۲۰۱۱، مرکز سلامت غذای اتحادیه اروپا که تنظیمکنندهی مقررات ملاتونین است، یک بررسی اجمالی بر روی نتایج مطالعات ملاتونین انجام داد و عنوان کرد: «ملاتونین به کاهش زمان به خواب رفتن کمک شایانی میکند.»
این نتیجه به نظر درست میآید اما همهی پژوهشگران بر سر اینکه ملاتونین میتواند تفاوت چشمگیری ایجاد کند، توافق ندارند. فیلیپ جرمن (Philip Gehrman)، پزشک و پژوهشگر خواب در دانشگاه پنسیلوانیا، میگوید: «مطالعات به روشنی نشان میدهند که ملاتونین یک قرص خواب طلوب است.»
یک بررسی جامع در سال ۲۰۰۵ با تحلیل ۱۵ پژوهش صورت گرفته توسط دارونما بر روی افرادِ دچار بیخوابی، به این مسئله نگاه دقیقتری داشت. این بررسی نشان داد که ملاتونین به طور متوسط زمان به خواب رفتن را در مقایسه با دارونما به مقدار ۳.۹ دقیقه کاهش و زمان خواب را ۱۳.۷ دقیقه افزایش میدهد و همچنین بازده خواب (نسبت زمان دراز کشیدن به زمان خواب) را تا ۳.۱ درصد بالا میبرد.
علاوه بر این، یک بررسی جامع در سال ۲۰۰۷ نشان داد که یک نوع قرص خواب به بنزودیازپین (benzodiazepines) به طور متوسط انسان را ۱۰ دقیقه زودتر به خواب میبرد. هنگامی که خواب هستید، این دارو زمان خواب شما را تا ۳۰ دقیقه و حتی افزایش میدهد. فراموش نکنید که این دارو میتواند اعتیادآورد باشد و در دوزهای بالا سبب مرگ مصرفکننده شود.
ساعت بدن فردی که پرواززده شده است از ساعت مقصد جا میماند و همین موجب میشود که فرد به سختی بخوابد.
ملاتونین ممکن است به ویژه برای پرواز زدهها مفید باشد چون هم قادر است به تغییر ساعت بدن برای مطابقت با ساعت محلی کمک کند و هم موجب به خواب رفتن پس از یک پرواز طولانی شود.
در سال ۲۰۰۲، کتابخانهی کوکرین یک مرور سیستماتیک بر روی قابلیتهای ملاتونین انجام داد که بر اساس آن ملاتونین قادر است به انسان کمک کند تا برهی جدید خواب خود را پس از پرواززدگی با دوزهایی به کوچکی ۰/۵ میلیگرم مجددا تنظیم کند. بر اساس نتایج حاصل از این تحقیق، ملاتونین در جلوگیری یا کاهش پرواززدگی به ویژه در پروازهای شرقی تاثیر بهسزایی دارد و استفادهی کوتاهمدت از آن بیخطر است. همچنین، ۸ مورد از ۱۰ آزمایش نشان دادهاند که اگر ملاتونین در زمانی نزدیک به زمان خواب مقصد (۱۰ شب تا نیمهشب) مصرف شود، پرواززدگی را در پروازهایی که از مناطق زمانی ۵ ساعت یا میگذرند کاهش میدهد.
به نظر میرسد که ملاتونین برای برخی افراد عملکرد بهتری دارد.
وارتمن میگوید «اگر مشکل خواب شما ناشی از کمبود هورمون ملاتونین باشد، مصرف ملاتونین برای رفع این مشکل بسیار موثر خواهد بود.» اگر منشأ مشکل خواب مثلا اضطراب باشد، مصرف این قرص آنچنان اثربخش نیست.
فرانک شیِر (Frank Scheer)، دانشجوی گاهزیستشناسی (chronobiology) در هاروارد، تصریح میکند که هنگام پایین بودن سطح ملاتونین در داخل بدن، دوز دهانی ملاتونین قابلیت بالایی در خوابآلود کردن فرد دارد. او میگوید: «در افراد سالم، اگر در میانهی روز ملاتونین مصرف شود، این کار به آنها کمک خواهد کرد که در میانهی روز به خواب بروند و اگر در شب ملاتونین مصرف شود، هیچ تاثیری بر خواب آنها نخواهد داشت.»
شیِر همچنین بیان میکند که ملاتونین از منحنی معمولِ واکنش-دوز (dose-response curve) را دنبال نمیکند. این یعنی مصرف دوز بالاترِ ملاتونین لزوما منجر به خواب نمیشود.
بدن انسان با افزایش سن ملاتونین کمتری تولید میکند، بنابراین این هورمون برای درمان بیخوابیِ افراد مسن مناسب است. ملاتونین همچنین بر افرادی که دچار اختلال تاخیر در فاز خواب (Delayed sleep phase disorder) (اختلال ناهمگامی دائمی ساعت بیولوژیکی بدن با محیط) هستند، تاثیر بیشتری دارد.
دویستر گزراشهای ۳۵ پژوهش در خصوص عوارض جانبی ملاتونین را بررسی کرده و چیز چشمگیری نیافته است. معمولترین عارضهی جانبی گزارش شده خوابآلودگی (که خاصیت اصلی ملاتونین است) و سردرد بوده است.
اما شیِر میگوید مواردی دیده شده که ملاتونین عوارض جانبی داشته است. ملاتونین توانایی بدن برخی افرادِ دارای یک تنوع ژنتیکی خاص را در پردازش گلوکز خون معیوب میسازد که البته به گفتهی او بیضرر هم نیست.
متاسفانه در بسیاری از مطالعات، خطرات استفادهی طولانیمدت مورد ارزیابی قرار نگرفته است. شیِر میگوید: «هیچ مطالعهی روشنی دربارهی مصرف ملاتونین برای بیش از ۶ ماه انجام نگرفته است.»
پژوهشگران هرگز یک بررسی سیستماتیک در خصوص اهمیت زمانبندی مصرف ملاتونین برای افرادی که میخواهند سریعتر به خواب بروند، انجام ندادهاند.
حتی پژوهشگرانی که در رابطه با این مقاله با اونها مصاحبه شد نیز بسته به اینکه فکر میکردند ملاتونین برای بهبود خواب بهتر است یا برای تنظیم ساعت شبانهروز، در خصوص زمان مصرف ملاتونین اتفاق نظر نداشتند.
بر اساس نظر جرمن، قابلیت ملاتونین در تغییر ساعت شبانهروزی بدن قویتر از قابلیت آن در سریع به خواب بردن است. به گفتهی او «به جای مصرف ملاتونین در زمان خواب، چند ساعت پیش از این زمان آن را مصرف کنید. در این صورت ساعت بیولوژیکی بدن هر روز حدود نیم ساعت به جلو میرود.»
شیِر تصریح میکند که مصرف ملاتونین در ساعتهای انتهایی بعدازظهر یا اول شب منجر به بیشترین پیشروی در فاز خواب میشود یعنی روز بعد ساعت بدن شما مقداری جلوتر خواهد بود.
او میگوید قرار گرفتن در معرض نور خورشید در صبح نیز به این موضوع کمک میکند و حتی کشیدن ساعت به جلو شاید موثرتر از ملاتونین باشید و گاهی باید در کنار مصرف ملاتونین این کار را انجام داد.
از طرف دیگر، وارتمن مصرف ملاتونین را چندین ساعت پیش از خواب توصیه نمیکند. او میگوید توانایی هرچند ضعیف ملاتونین در فراهم آوردن خوابِ بهموقع این معنا را دارد که این دارو باید زمان کوتاهی پیش از خواب مصرف شود.
در مجموع به نظر شیِر، پیشبینی زمان ایدهآل مصرف، کاری دشوار است. به گفتهی او قاعدهی مصرف ملاتونین ساده نیست و بسیاری از پزشکان شاید از طریقهی مصرف صحیح آن آگاهی نداشته باشند.
جولیا بلوز (Julia Belluz) بر روی گزارشهای مفصلی دربارهی قوانین مبهم مکملهای رژیمی در آمریکا کار کرده است. او مکملهای رژیمی را به یک راهروی پر از مین تشبیه کرده است. محتویات مکملها معمولا شامل آن چیزی که میگویند نیستند و گاهی اوقات دارای مواد افزودنی مضر هم هستند. بنابراین میباید اطمینان حاصل کنید که محتویات برند ملاتونینی که خریداری میکنید توسط یک نهاد ثالث که نقش نظارتی دارد (مثل وزارت بهداشت) تایید شود. بر اساس نظر وارتمن، این فرض که ملاتونین یک مکمل رژیمی است به کل اشتباه است و ملاتونین طبیعیِ بدن ما اصلا از طریق رژیم غذایی تامین نمیشود.
در خصوص ملاتونین، اهمال دیگری که در قانون وجود دارد امکان خرید دوزهای بالا و غیرضروری است. در داروخانهها به راحتی میتوان قرص ملاتونین ۱۰ میلیگرمی تهیه کرد که اصلا مصرف آن کمکی نخواهد کرد. مطالعات نشان میدهند که مصرف دوزهای پایین، مثلا ۱ میلیگرم یا کمتر، تاثیر یکسانی با دوزهای بالا دارد. علاوه بر این، مصرف دوزهای بالا خطر بیحسی مغز نسبت به هورمون را در پی خواهد داشت. وارتمن میگوید: «اگر شما میزان ملاتونین خون مغز خود را بسیار از حد نرمال بالا ببرید، دریافتکنندههای مغز نسبت به ملاتونین بیحس میشوند و واکنش بدن شما ضعیف و ضعیفتر خواهد شد.» در نتیجه توصیهی کارشناسان بر این است که مصرف ملاتونین را باید با دوزهای پایینتر شروع کرد.
و در نهایت، دانستن این نکته مهم است که قرص هرگز به تنهایی نمیتواند مشکلات و اختلالات خواب را برطرف کند. دلایل بسیاری زیادی در دنیای مدرن امروز سد راه خواب خوب و صحیح هستند: استرس در محیط کار و خانه، همسایگان پرسروصدا، استفاده بسیار کم از نور روز، مصرف زیاد کافئین، کار کردن در شیفتهای غیرمعمول، ورزش نکردن، استفاده از تلفنهای هوشمند تا دیروقت و .

سلام. من آدم دق خور حساس و زودرنجی ام. دربچگی مادربزرگ وعموم ودخترخاله م رو ازدست دادم که به مادربزرگم وعموم خیییییییییییییییییلی وابسته بودم به این دلیل که طبقه پایین منزل اونها زندگی می کردیم مشکلات این ضربه روحی واتفاقات بعدش و..از من آدمی شکننده با روحیه ای بسیار شکننده ساخته مشکلی هم اخیرا داشتم که به خاطرش دلم نمی خواست حتی زنده باشم برای همین سر خود آلپروزولام ۴ تا ۴ تا مصرف می کردم تا بخوابم و هیچ فکری نیاد توذهنم. الان چند روزیه که دیگه قرص نمی خورم وبه جاش عرق چهل گیاه مصرف می کنم اوایل خوابم می برد ولی الان تا ۶ صبح بیدارم و دکتر برام ملاتونین تجویز کرده آیا می تونم از یکی بخورم؟

دز بالاتر ملاتونین لزوما باعث خواب نمیشه. علاوه بر این، ممکنه دچار عارضهی جانبی اون یعنی سردرد هم بشید. یا مغز شما نسبت به گیرندههای ملاتونین بیحس بشه و واکنش بدنتون ضعیف و ضعیفتر بشه. بهتره دارو رو طبق دستور پزشک مصرف کنید.

برخی افراد فارغ از اینکه چه میکنند، با خواب شب و بیدارماندن در طول روز مشکل دارند. این مسئله ممکن است برای ایشان مشکلساز باشد یا نباشد، بستگی به سبک زندگی و کار یا برهی تحصیلیشان دارد. اگر شما هم از آن شبزندهدارها هستید، نکتههایی هستند که میتوانید امتحان کنید تا شاید زودتر بخوابید.
زود بیدار شدن هر روز در ساعت مشخصی، فارغ از اینکه چه ساعتی خوابیدهاید: آخرهفتهها (یا روزهایی که مجبور نیستید از رختخواب بلند شوید)، به خودتان اجازه ندهید بیش از ۱ ساعت طولانیتر از وقتی که مجبورید به محل کار یا مدرسه بروید، بخوابید. اگر این شیوه برای شما اثری ندارد، میتوانید این درمانها را هم امتحان کنید:
نور درمانی: در این مورد، نور درمانی یعنی به محض اینکه بیدار شدید، خودتان را در معرض روشنایی قرار دهید. میتوانید از نور خورشید یا جعبهی نور (لایتباکس) به مدت ۳۰ تا ۴۵ دقیقه روزانه استفاده کنید.
ملاتونین: از پزشک خود بپرسید آیا میتوانید شبها برای به خوابرفتن، از ملاتونین مصرف کنید یا نه.
کرونو تراپی: این روش برای شبزندهدارها، بهمعنای ایجاد روز ۲۷ ساعته است. در این روش، طی هر چرخهی خواب و بیداری، ۳ ساعت دیرتر به خواب میروید تا این چرخه به زمانی برسد که میخواهید در آن ساعت به خواب بروید. پس از اینکه این چرخه کامل شد، دیگر در آن ساعت مطلوب میخوابید. اجرای این روش میتواند دشوار باشد، آن هم به خاطر اینکه ممکن است برهی روزمرهتان را مختل کند. این یک برهی زمانی نمونه است:

Melatonin is the natural hormone your body secretes that helps to maintain your wake-sleep cycle (also called “biological clock). The wake-sleep cycle is the process of sleep and wakefulness in humans this averages 8 hours of nighttime sleep and 16 hours of daytime activity. Melatonin is also made synthetically and available without a preion as an over-the-counter (OTC) dietary supplement in the U.S.
Endogenous melatonin release (melatonin made by our bodies) is increased each day in response to darkness, peaking between 11PM and 3AM at roughly 2 picograms (pg) per mL. Nighttime melatonin hormone levels are roughly 1 times higher than in the daytime. Levels fall sharply before daylight, and are barely detectable in the daylight hours. The rise and fall in endogenous melatonin levels signal wake and sleep times, known as our circadian rhythm.
Natural melatonin secretion starts from the amino acid tryptophan, with serotonin as an intermediary, and then is released to the melatonin receptors in the brain, eye and other areas to help control the sleep and wake cycles. The melatonin half-life is short, roughly 2 to 5 minutes. It is metabolized (broken down) by the CYP-45 enzyme system in the liver and then excreted in the urine or feces.
Shorter periods of melatonin production occur in the summer with longer days, and more prolonged periods of production occur in the winter. Light at night (such as from smartphones or the TV) blocks the production of melatonin and can lead to sleep disturbances. Age also suppresses the levels of nighttime melatonin that are released, which may contribute to the problem of insomnia and early awakening often seen in older adults.
Melatonin supplementation has been suggested to have many uses, from sleep disorders to cancer treatment , but robust studies are lacking for many uses. However, it has been widely studied for treatment of jet lag and other sleep disorders. Additional, early research to define melatonin suggests it has an anti-oxidative activity, a role in modulating immune responses, and possible anti-tumor activity.
Melatonin is a natural hormone when produced in the body (the endogenous hormone). Melatonin is not a vitamin. Melatonin supplements (exogenous hormone) are made synthetically and all products and strengths on the U.S. market are available without a preion at the pharmacy, nutrition stores, and other retail shops.

Natural melatonin is a highly lipid soluble hormone produced in the pineal gland in the brain. It is synthesized from the amino acid tryptophan and then released into the blood and cerebrospinal fluid, crossing the blood-brain barrier. It sends messages to the melatonin receptor agonist in the brain and other areas of the body to help control the sleep and wake cycles.
Does melatonin help you sleep? When taken as supplement, the of melatonin is to mimic the effects of the natural hormone. Drowsiness generally occurs within 3 minutes after taking melatonin. Melatonin is considered an effective treatment for jet lag and can aid sleep during times when you would not normally be awake.
Effective starting doses for jet lag range from .3 to .5 mg. One milligram tablets can be cut in half to achieve a .5 mg dose if smaller doses are not available for purchase. Lower doses may work for some people, while others may need a higher dose, up to 3 to 5 mg. However, higher doses may be associated with more side effects such as ache, next day grogginess, or vivid dreams.
Always start with the lowest melatonin dose. According to a Cochrane review, doses over 5 mg appear to be no more effective than lower doses. It is important to note that much higher doses are available for sale in the U.S., but these doses may result in excessively high levels of physiologic melatonin.
Jet lag is an air travel problem that causes difficulty in sleeping, fatigue, trouble concentrating, constipation, and other symptoms. Jet lag is much more likely if you cross over several time zones, and can worsen the more time zones you cross.
Effective starting doses for jet lag range from .3 to .5 mg. Smaller doses may work for some while others may need a higher dose. It may be more difficult to fly east, when time is lost, rather than to fly west, when you gain it back. High doses, such as 2 mg melatonin is available for purchase on the Internet, but such high doses are not normally recommended or needed.
Given enough time (usually 3 to 5 days), jet lag will usually resolve on its own, but this is not always optimal when traveling.
How to Take Melatonin for Sleep (Insomnia):
Dosage: Take melatonin .1 mg to .5 mg thirty minutes before bedtime. Studies suggest melatonin for sleep may be effective in promoting but not maintaining sleep (early morning awakening).
How to Take Melatonin for Shift-Work Sleep Disorders
Dosage: Take melatonin 1.8 mg to 3 mg thirty minutes prior to the desired onset of daytime sleep melatonin may NOT lead to improved alertness during the nighttime work shift and may only improve daytime sleep time by about 3 minutes.
How to Take Melatonin for Delayed Sleep Phase Disorder
Delayed sleep phase disorder (DSPD) most often occurs in adolescents, possibly due to reduced melatonin production and melatonin deficiency at this age. Sleep onset is delayed by 3 to 6 hours compared with conventional bedtimes (1 to 11 pm). DSPD can negatively affect school performance, daily activities, and lead to morning drowsiness which can be dangerous for teen drivers. Any sleep disorder in an adolescent should be evaluated by a physician.
Dosage: Take melatonin 1 mg four to six hours before set bedtime. Once a set bedtime is achieved, use maintenance doses of .5 mg melatonin 2 hours before expected sleep onset. Bright light therapy and behavioral management may enhance results. Be aware drowsiness may occur after melatonin dose, so avoid hazardous activities such as driving.
How to Take Melatonin for Non-24-Hour Sleep Wake Disorder (Non-24)
More than 7% of people who are totally blind have Non-24, a circadian rhythm disorder. For people who are totally blind, there are no light cues to help reset the biological clock. The sleep time and wake up time of people who have Non-24-Hour Sleep Wake Disorder shifts a little later every day. Sleep times go in and out of alignment compared to a normal sleep-wake phase. Extra minutes add up each day by day and disrupt the normal wake-sleep pattern.
Use of melatonin in Non-24 is to aid in stimulation to reset the biological clock with one long sleep time at night and one long awake time during the day. However, no large-scale clinical trials of melatonin therapy for Non-24 have been conducted to date.
Dosage: Studies on the blind suggest that .5 mg/day melatonin is an effective dose.
Hetlioz, a preion-only melatonin agonist is also approved for use in Non-24-Hour Sleep Wake Disorder in blind individuals.

Fast-dissolving Melatonin
Some melatonin tablets are available in fast-dissolving formulations in the U.S. To take the orally disintegrating tablet:

Parents may consider using melatonin to help their child who has a trouble falling asleep. Only use melatonin for your child under the care of a pediatrician or other medical sleep specialist. Insomnia or other sleeping disorders in children should always be evaluated by a medical professional.
Melatonin should not be used as a substitute for good sleep hygiene and consistent bedtime routines in children. According to Yale sleep specialist Dr. Craig Canapari, use of melatonin results in less difficulty with falling asleep, earlier time of sleep onset, and more sleep at night.
Products containing lower-dose melatonin for kids do exist on the U.S. market. However, long-term use of melatonin has not been studied in children and possible side effects with prolonged use are not known. Use for children with autism spectrum disorder or attention-deficit hyperactivity disorder should involve behavioral interventions and should be directed by a physician.
Delayed sleep phase disorder often occurs in teenagers and young adults, possibly due to alterations in endogenous melatonin production. Sleep onset is delayed by 3 to 6 hours compared with normal bedtime hours of 1 to 11 PM. Maintaining a consistent bedtime free of electronics for at least one hour prior to bedtime is especially important for children and adolescents.

Dietary melatonin supplements can still have drug interactions or health risks if you have certain medical conditions, upcoming surgery, or other health concerns.

If you miss taking a dose of melatonin, there is no cause for concern. However, melatonin may impair your thinking and reaction time. If you will be driving or doing any hazardous activity, skip your dose as melatonin can cause drowsiness.
Otherwise, use the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not use extra medicine or double your dose to make up the missed medication.
Melatonin is thought to be very safe in the short-term with a low risk for overdose. However, if you suspect a melatonin overdose, seek emergency medical attention or call the Poison Help Line at 1-8-222-1222.
Melatonin is a relatively safe supplement when used in the short-term, and melatonin side effects are uncommon. Melatonin safety in the long-term has not been determined in randomized, controlled studies. In general melatonin appears to be relatively nontoxic, even at higher doses such as 3 to 5 mg once a day.

Melatonin is not approved by the FDA for any use. Melatonin falls under the FDA’s Dietary Health and Education Act as a dietary supplement. The FDA is not authorized to review the safety or effectiveness of dietary supplements like melatonin before they are marketed.
However, manufacturers must notify FDA about new ingredients prior to marketing. The FDA will only review (not approve) the ingredient for safety, but not effectiveness. If a manufacturer makes an unproven health claim or if the supplement is found to be unsafe the FDA can remove the OTC from the market.
Avoid buying dietary supplements from the Internet and from online pharmacies whose authenticity you cannot verify. It is important to remember that an OTC label that says “natural melatonin supplement does not always mean it’s “safe for consumption. Formulations that are United States Pharmacopeial (USP) Convention Verified can be considered most reliable in this regard the label “USP is found on the outside of the bottle. Light at night blocks the production of melatonin, and this has been shown to cause sleep disturbances in people who use electronics that emit light at night. Researchers have found that light from electronic devices can block the release of natural hormone melatonin at night. The effect was most significant for younger children, with nighttime melatonin levels reduced by up to 37 percent in certain cases. This can also be problematic for children between ages 9 and 16.
Studies show that light after dark lowers melatonin levels which may lead to difficulty in initiating sleep. Electronic light-emitting devices such as the television, laptops, smartphones and tablets often find their way into the bedroom at night, or are used after dark. Children should not have electronics, including TVs, in their bedroom at bedtime and everyone should avoid use of light-emitting devices and screen time at least one hour before bedtime.
Researchers have conducted many studies on melatonin supplements for ious conditions. Most studies have been conducted in sleep disorders, such as jet lag, shift work sleep disorders, delayed sleep phase disorder, and insomnia. However, studies are often not consistent in their results and questions still remain about its usefulness, dosage, length of treatment and long-term safety for some sleep conditions.
Melatonin can be effective for jet lag for many people when dosed at the appropriate time. Melatonin effectiveness for insomnia might slightly hasten the amount of time needed to fall asleep, but may not increase the overall sleep time. Melatonin does appear to be safe for short-term use (less than three months).
Multiple areas for melatonin use have been investigated. However, not all trials are of adequate size or quality to make final conclusions about use of melatonin in these conditions:
In the U.S., melatonin pills can be purchased without a preion in the pharmacy, grocery, or health food store. Melatonin strengths range from 1 milligram (mg) to 1 mg melatonin, but you should always start with the lowest dose to judge its effects. Research finds that melatonin synthesis depends upon availability of the essential amino acid tryptophan, a needed component of the diet. If intake of tryptophan is severely restricted, synthesis of melatonin is significantly reduced in humans.
