This leaflet answers some common questions about Dostinex. It does not contain all the available information. It does not take the place of talking to your doctor or pharmacist.

All medicines have risks and benefits. Your doctor has weighed the risks of you using Dostinex against the expected benefits.

If you have any concerns about using this medicine, ask your doctor or pharmacist.

You may need to read it again.

Dostinex may be prescribed by your doctor to reduce your body’s level of a hormone known as prolactin. Dostinex may be needed if your levels of prolactin are abnormally high (hyperprolactinaemia).

Abnormally high prolactin may cause menstrual changes in women, impotence in men and breast changes in both sexes.

Dostinex can also be prescribed to prevent the production of milk in women after birth, if breastfeeding is to be prevented for medical reasons.

Ask your doctor if you have any questions about why this medicine has been prescribed for you.

Your doctor may have prescribed it for another reason.

Dostinex is available only with a doctor’s prescription.

Before starting treatment with Dostinex, your doctor will need to do some tests to detect any underlying heart, lung or kidney disease.

These tests include chest x-rays, physical examinations, blood tests and heart monitoring.

Your doctor will repeat these tests regularly if you are taking Dostinex for a long period of time.

While you are taking Dostinex report anything unusual to your doctor such as difficulty in breathing, chest pain, swelling of your hands or feet, or anything else that is making you feel unwell.

Do not take Dostinex if you have an allergy to:

Some of the symptoms of an allergic reaction may include shortness of breath, wheezing or difficulty breathing; swelling of the face, lips, tongue or other parts of the body; rash, itching or hives on the skin.

Do not take Dostinex if you have or have had:

Do not take this medicine if you are pregnant.

It may affect your developing baby if you take it during pregnancy.

Do not give this medicine to a child under the age of 16 years.

The safety and effectiveness in children has not been established.

Do not take this medicine after the expiry date printed on the pack or if the packaging is torn or shows signs of tampering.

If it has expired or is damaged, return it to your pharmacist for disposal.

If you are not sure whether you should start taking this medicine, talk to your doctor.

Tell your doctor if you have or have had any of the following medical conditions:

Tell your doctor if you are pregnant or plan to become pregnant.

Your doctor can discuss with you the risks and benefits involved. It is recommended that women who plan to become pregnant stop taking Dostinex at least one month before becoming pregnant.

Tell your doctor if you are breastfeeding or intend to breastfeed.

This medicine prevents the flow of breast milk.

If you have not told your doctor or pharmacist about any of the above, tell them before you start to take Dostinex.

Tell your doctor if you are taking any other medicines, including medicines you buy without a prescription from a pharmacy, supermarket or health food shop.

Some medicines and Dostinex may interfere with each other. These include:

These medicines may be affected by Dostinex or may affect how well it works. You may need different amounts of your medicine or you may need to take different medicines.

Your doctor or pharmacist has more information on medicines to be careful with or avoid while taking Dostinex.

Follow all directions given to you by your doctor or pharmacist carefully.

The directions given to you by your doctor or pharmacist on how to take Dostinex may differ from the information contained in this leaflet. You may be given a different dosage depending on your condition or how you react to the medicine.

If you do not understand the instructions on the label, ask your doctor or pharmacist for help.

To treat high levels of prolactin (hyperprolactinaemia), the recommended starting dose is half a tablet taken twice a week. Your doctor will generally start you on a low dose and may gradually increase your dose.

To prevent the production of breast milk, the recommended dose is two tablets taken as a single dose, on the first day after delivery of your baby.

Swallow Dostinex tablets with a glass of water.

Take your medicine with food or a meal.

Continue taking Dostinex until your doctor tells you to stop.

If it is almost time for your next dose, skip the dose you missed and take your next dose when you are meant to.

Otherwise, take it as soon as you remember, then go back to taking your medicine as you normally would.

Do not take a double dose to make up for the dose that you missed.

This may increase the chance of you getting an unwanted side effect.

If you are not sure what to do, speak to your doctor or pharmacist.

If you have trouble remembering to take your medicine, ask your pharmacist for some hints.

Immediately telephone your doctor or the Poisons Information Centre for advice (in Australia telephone 13 11 26, in New Zealand telephone 0800 POISON or 0800 764 766) or go to the Accident and Emergency department (Casualty) at your nearest hospital if you think you or anyone else may have taken too much Dostinex. Do this even if there are no signs of discomfort or poisoning.

You may need urgent medical attention. Keep telephone numbers for these services handy. Have the medicine or this leaflet available to give details if needed.

The symptoms of taking too much Dostinex may include nausea, vomiting, stomach pains and dizziness.

Follow your doctor’s requests for tests and report anything unusual to your doctor such as difficulty in breathing, chest pain or swelling of your hands or feet.

Keep all of your doctor’s appointments while taking Dostinex so that your progress can be checked.

It is important that your doctor carries out some regular tests if you are taking Dostinex for a long period of time to make sure the medicine is working and to prevent unwanted side effects. These tests may include chest x-rays, physical examinations, blood tests and heart monitoring.

If you are about to be started on any new medicine, remind your doctor and pharmacist that you are taking Dostinex.

Tell any other doctors, dentists, and pharmacists who treat you that you are taking this medicine.

Use barrier methods of contraception, such as condoms, to prevent pregnancy during and for at least one month after taking Dostinex.

Your doctor may recommend routine pregnancy tests during long periods of treatment.

Stop taking Dostinex immediately if you become pregnant and consult your doctor.

Do not use Dostinex to treat any other medical complaints unless your doctor tells you to.

Do not stop taking your medicine until your doctor tells you to, even if you are feeling better.

Do not give Dostinex to anyone else, even if they have the same condition as you.

If you feel light-headed, dizzy or faint when getting out of bed or standing up, get up slowly.

Be careful driving or operating machinery until you know how Dostinex affects you.

Dostinex may cause dizziness or affect your ability to respond quickly.

If you feel sleepy while taking Dostinex, do not drive or operate machinery.

See your doctor if you notice changes in your behaviour that result in a strong desire to either gamble, shop, eat or use medicines to excess, or you notice an increase in your sex drive.

Such compulsive behaviours have been seen with this class of medicine, including cabergoline.

Tell your doctor as soon as possible if you do not feel well while taking Dostinex, even if you do not think the problems are connected with the medicine or they are not listed in this leaflet.

Like many medicines, Dostinex may cause side effects. If they occur, they are likely to be minor and temporary. However, some may be serious and need medical attention.

It can be difficult to tell whether side effects are the result of taking Dostinex, effects of your condition or side effects of other medicines you may be taking. For this reason it is important to tell your doctor of any change in your condition.

Do not be alarmed by the following list of possible side effects.

You may not experience any of them.

Ask your doctor or pharmacist about any questions you may have.

Tell your doctor if you notice anything else that is making you feel unwell when you are taking, or soon after you have finished taking Dostinex.

Other side effects not listed above may also occur in some patients.

Some of these side effects (e.g. changes in liver function) can only be found when your doctor does tests from time to time to check your progress.

Keep Dostinex in its container until it is time to take it.

Keep Dostinex in a cool dry place, where the temperature stays below 25°C.

Do not store Dostinex, or any medicines, in a bathroom or near the sink. Do not leave Dostinex in the car or on window sills.

Heat and dampness can destroy some medicines.

Keep Dostinex where children cannot reach it.

A locked cupboard at least one-and-a-half metres above the ground is a good place to store medicines.

Ask your pharmacist what to do with any Dostinex which is left over, or if the medicine has passed its expiry date.

Dostinex comes as capsule-shaped, flat, white tablets. One side is marked with the letter ‘P’ on a side of the score and the letter ‘U’ on the other. The other side of the tablet is marked ‘700’ with a short score on the top and bottom of the tablet surface.

Dostinex is available in amber glass bottles of 2 and 8 tablets.

Active Ingredient The active ingredient is cabergoline 500 microgram.

Inactive Ingredients Each tablet also contains: lactose and leucine.

Cabergoline, an ergot derivative, is a long-acting dopamine agonist and prolactin inhibitor. It is used to treat hyperprolactinemic disorders and Parkinsonian Syndrome. Cabergoline possesses potent agonist activity on dopamine D2 receptors.

For the treatment of hyperprolactinemic disorders, either idiopathic or due to prolactinoma (prolactin-secreting adenomas). May also be used to manage symptoms of Parkinsonian Syndrome as monotherapy during initial symptomatic management or as an adjunct to levodopa therapy during advanced stages of disease.

Cabergoline stimulates centrally-located dopaminergic receptors resulting in a number of pharmacologic effects. Five dopamine receptor types from two dopaminergic subfamilies have been identified. The dopaminergic D1 receptor subfamily consists of D1 and D5 subreceptors, which are associated with dyskinesias. The dopaminergic D2 receptor subfamily consists of D2, D3 and D4 subreceptors, which are associated with improvement of symptoms of movement disorders. Thus, agonist activity specific for D2 subfamily receptors, primarily D2 and D3 receptor subtypes, are the primary targets of dopaminergic antiparkinsonian agents. It is thought that postsynaptic D2 stimulation is primarily responsible for the antiparkinsonian effect of dopamine agonists, while presynaptic D2 stimulation confers neuroprotective effects. This semisynthetic ergot derivative exhibits potent agonist activity on dopamine D2- and D3-receptors. It also exhibits: agonist activity (in order of decreasing binding affinities) on 5-hydroxytryptamine (5-HT)2B, 5-HT2A, 5-HT1D, dopamine D4, 5-HT1A, dopamine D1, 5-HT1B and 5-HT2C receptors and antagonist activity on α2B, α2A, and α2C receptors. Parkinsonian Syndrome manifests when approximately 80% of dopaminergic activity in the nigrostriatal pathway of the brain is lost. As this striatum is involved in modulating the intensity of coordinated muscle activity (e.g. movement, balance, walking), loss of activity may result in dystonia (acute muscle contraction), Parkinsonism (including symptoms of bradykinesia, tremor, rigidity, and flattened affect), akathesia (inner restlessness), tardive dyskinesia (involuntary muscle movements usually associated with long-term loss of dopaminergic activity), and neuroleptic malignant syndrome, which manifests when complete blockage of nigrostriatal dopamine occurs. High dopaminergic activity in the mesolimbic pathway of the brain causes hallucinations and delusions; these side effects of dopamine agonists are manifestations seen in patients with schizophrenia who have overractivity in this area of the brain. The hallucinogenic side effects of dopamine agonists may also be due to 5-HT2A agonism. The tuberoinfundibular pathway of the brain originates in the hypothalamus and terminates in the pituitary gland. In this pathway, dopamine inhibits lactotrophs in anterior pituitary from secreting prolactin. Increased dopaminergic activity in the tuberoinfundibular pathway inhibits prolactin secretion.

The dopamine D2 receptor is a 7-transmembrane G-protein coupled receptor associated with Gi proteins. In lactotrophs, stimulation of dopamine D2 causes inhibition of adenylyl cyclase, which decreases intracellular cAMP concentrations and blocks IP3-dependent release of Ca2+ from intracellular stores. Decreases in intracellular calcium levels may also be brought about via inhibition of calcium influx through voltage-gated calcium channels, rather than via inhibition of adenylyl cyclase. Additionally, receptor activation blocks phosphorylation of p42/p44 MAPK and decreases MAPK/ERK kinase phosphorylation. Inhibition of MAPK appears to be mediated by c-Raf and B-Raf-dependent inhibition of MAPK/ERK kinase. Dopamine-stimulated growth hormone release from the pituitary gland is mediated by a decrease in intracellular calcium influx through voltage-gated calcium channels rather than via adenylyl cyclase inhibition. Stimulation of dopamine D2 receptors in the nigrostriatal pathway leads to improvements in coordinated muscle activity in those with movement disorders. Cabergoline is a long-acting dopamine receptor agonist with a high affinity for D2 receptors. Receptor-binding studies indicate that cabergoline has low affinity for dopamine D1, α1,- and α2- adrenergic, and 5-HT1- and 5-HT2-serotonin receptors.

Hepatic. Cabergoline is extensively metabolized, predominately via hydrolysis of the acylurea bond of the urea moiety. Cytochrome P-450 mediated metabolism appears to be minimal. The main metabolite identified in urine is 6-allyl-8b-carboxy-ergoline (4-6% of dose). Three other metabolites were identified urine (less than 3% of dose).

After oral dosing of radioactive cabergoline to five healthy volunteers, approximately 22% and 60% of the dose was excreted within 20 days in the urine and feces, respectively. Less than 4% of the dose was excreted unchanged in the urine.

The elimination half-life is estimated from urinary data of 12 healthy subjects to range between 63 to 69 hours.

Overdosage might be expected to produce nasal congestion, syncope, or hallucinations.

Cabergoline is used to treat different types of medical problems that occur when too much of the hormone prolactin is produced. It can be used to treat certain menstrual problems, fertility problems in men and women, and pituitary prolactinomas (tumors of the pituitary gland).

It works by stopping the brain from making and releasing the prolactin hormone from the pituitary gland. Cabergoline use is usually stopped when prolactin levels are normal for 6 months. It may be given again if symptoms of too much prolactin occur again.

Cabergoline oral tablet is a prescription drug that’s only available as a generic drug. There is no brand-name version available. Cabergoline only comes as a tablet you take by mouth.

Cabergoline is used to treat hyperprolactinemia (high levels of prolactin in your body). This happens when your pituitary gland makes extra prolactin. High prolactin levels may be caused by a hormone problem or by a tumor in your pituitary gland.

High prolactin levels can cause changes in a woman’s ovulation, menstrual cycle, and breast milk production. In men, high prolactin levels can affect reproduction and cause sexual issues. These issues include a decreased sex drive and not being able to get or keep an erection.

Cabergoline belongs to a class of drugs called dopamine agonists. A class of drugs is a group of medications that work in a similar way. These drugs are often used to treat similar conditions.

Dopamine is a chemical your body makes. It prevents your body from releasing prolactin. Cabergoline works by binding to dopamine receptors. This also keeps your body from releasing prolactin, which helps decrease your prolactin levels.

The more common side effects of cabergoline oral tablet can include:

Cabergoline can cause a severe allergic reaction. Symptoms can include:

If you have an allergic reaction, call your doctor or local poison control center right away. If your symptoms are severe, call 911 or go to the nearest emergency room. Don’t take this drug again if you’ve ever had an allergic reaction to it. Taking it again could be fatal (cause death).

For people with heart problems: If you have uncontrolled high blood pressure or heart valve problems, you should not use this drug. It can make your condition worse. If you have high blood pressure, ask your doctor if your blood pressure is under control.

For people with history of tissue scarring: If you have a history of scarred tissue in your lungs, heart, kidney, or abdomen (stomach area), you should not use this drug. It can make your condition worse.

For people with high blood pressure from pregnancy: If you’re currently pregnant and have high blood pressure from your pregnancy, you should not use this drug. It can make your condition worse.

For people with liver problems: You may not be able to process this drug well. This may increase the levels of the drug in your body and cause more side effects.

For pregnant women: Cabergoline is a category B pregnancy drug. That means two things:

Talk to your doctor if you’re pregnant or plan to become pregnant. Animal studies do not always predict the way humans would respond. Therefore, this drug should only be used in pregnancy if clearly needed.

Call your doctor right away if you become pregnant while taking this drug.

For women who are breastfeeding: It isn’t known if this drug passes into breast milk. If it does, it may cause side effects in a child who is breastfed. This drug may also keep you from making breast milk. Talk to your doctor if you breastfeed your child. You may need to decide whether to stop breastfeeding or stop taking this medication.

For seniors: The kidneys and liver of older adults may not work as well as they used to. This can cause your body to process drugs more slowly. As a result, more of a drug stays in your body for a longer time. This raises your risk of side effects.

For children: This medication has not been studied in children. It should not be used in people younger than 18 years.

This dosage information is for cabergoline oral tablet. All possible dosages and drug forms may not be included here. Your dosage, drug form, and how often you take the drug will depend on:

Generic: Cabergoline

Adult dosage (ages 18 to 64 years)

Child dosage (ages 0 to 17 years)

This medication has not been studied in children. It should not be used in people younger than 18 years.

Senior dosage (ages 65 years and older)

The kidneys and liver of older adults may not work as well as they used to. This can cause your body to process drugs more slowly. As a result, more of a drug stays in your body for a longer time. This raises your risk of side effects.

Your doctor may start you on a lowered dosage or a different treatment schedule. This can help keep levels of this drug from building up too much in your body.

Dostinex is used to treat a hormone imbalance in which there is too much prolactin in the blood (also called hyperprolactinemia).

Dostinex may also be used for purposes not listed in this medication guide.

You should not use Dostinex if you have uncontrolled high blood pressure, a heart valve disorder, or a condition called fibrosis (growth of excess tissue in or around internal organs). You should not use this medicine if you are allergic to any type of ergot medicine.

You should not use Dostinex if you are allergic to it, or if you have:

uncontrolled high blood pressure (hypertension);

a heart valve disorder;

a growth of excess tissue (fibrosis) in your lungs or around your heart or stomach; or

an allergy to any type of ergot medicine, such as dihydroergotamine, ergotamine, ergonovine, or methylergonovine.

Tell your doctor if you have ever had:

liver disease;

heart problems; or

hypertension caused by pregnancy, including eclampsia and preeclampsia;

It is not known whether Dostinex will harm an unborn baby. Tell your doctor if you are pregnant or if you become pregnant.

You should not breast-feed while using Dostinex.

Your doctor will perform tests to make sure you do not have conditions that would prevent you from safely using Dostinex.

Follow all directions on your prescription label and read all medication guides or instruction sheets. Your doctor may occasionally change your dose. Use the medicine exactly as directed.

Dostinex is usually taken twice each week for at least 6 months. Do not take this medicine every day unless your doctor tells you to.

Your blood will need to be tested on a regular basis to measure your prolactin levels.

Store at room temperature away from moisture and heat.

Call your doctor for instructions if you miss a dose.

Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.

Overdose symptoms may include stuffy nose, hallucinations, or fainting.

Avoid getting up too fast from a sitting or lying position, or you may feel dizzy.

Get emergency medical help if you have signs of an allergic reaction: hives; difficult breathing; swelling of your face, lips, tongue, or throat.

Call your doctor at once if you have:

shortness of breath (even while lying down);

chest pain, dry cough or hack;

a light-headed feeling, like you might pass out;

pain in your side or lower back;

little or no urination; or

swelling in your ankles or feet.

You may have increased sexual urges, unusual urges to gamble, or other intense urges while taking Dostinex. Talk with your doctor if this occurs.

Common side effects may include:

nausea, constipation;

feeling weak or tired;

headache;

dizziness; or

drowsiness.

Using Dostinex with other drugs that make you drowsy can worsen this effect. Ask your doctor before using opioid medication, a sleeping pill, a muscle relaxer, or medicine for anxiety or seizures.

medicine to treat anxiety, mood disorders, or mental illness such as schizophrenia.
