آمی تریپتیلین
این دارو متعلق به گروه ضد افسردگی های سه حلقه ای می باشد. منظور از سه حلقه ای این است که در مولکول دارو سه حلقه به هم متصل هستند. مورد اصلی تجویز این دارو از گذشته تا کنون در درمان افسردگی بوده اما امروزه در برخی از درد های عصبی و جهت پیشگیری از میگرن و مواردی از این دست توسط پزشکان تجویز می شود .
توصیه های امی تریپتیلین
پیش از شروع مصرف این دارو در صورت باردار یا شیرده بودن ،سابقه ی بیماری هایی نظیر تیروئید پر کار ،مشکلات کبدی ،صرع، دیابت، یبوست های مزمن، بیماری قلبی عروقی، چشمی و یا بیماری های اعصاب و سایر بیماری هارا به پزشک خود اطلاع دهید.
از آنجا که این دارو بر روی سیستم عصبی تاثیر دارد، مصرف آن باید به صورت مداوم و تحت نظر پزشک باشد.اگرچه ممکن است در چند هفته ی ابتدایی درمان احساس بهبودی در شما ایجاد نشود، با این حال درمان خودرا ادامه دهید و طبق برنامه ی منظم به پزشک خود مراجعه کنید و همچنین برای بهبود روند درمان کلیه ی مشکلات و سوالات خودرا از پزشک یا داروساز خود سوال بفرمایید .
قطع این دارو به صورت ناگهانی و بدون نظر پزشک، می تواند عوارض نامناسبی برای بیمار در برداشته باشد که اصطلاحا به آن سندرم محرومیت دارو می گویند و عوارضی همچون سردرد، تهوع، ضعف، حملات ترس و بی حالی در بر دارد.از آنجا که این دارو می تواند در عملکرد داروهای دیگر در بدن اختلال ایجاد کند لازم است پیش از شروع مصرف لیستی از کلیه ی داروهای مصرفی خود (خصوصا سایر داروهایی که تاکنون و در یک ماه اخیر برای درمان افسردگی و خواب استفاده نموده اید) در اختیار پزشک یا داروسازتان قرار دهید.اگر آمی تریپتیلین به صورت یک نوبت در روز تجویز شود بهترین زمان مصرف آن شب ها پیش از خواب می باشد. با این حال دارو را همانطور که پزشکتان برای شما تجویز کرده مصرف نمایید.ممکن است پزشک شما درمان را با مقدار داروی کم شروع کند و پس از عادت کردن بدن، مقدار داروی شمارا افزایش دهد تا احتمال بروز عوارض ناشی از دارو کمتر شود.این دارو با غذا تداخلی ندارد و می توانید آن را قبل یا بعد از غذا مصرف نمایید.به دلیل این که مسمومیت با ضد افسردگی های سه حلقه ای بسیار خطرناک است دارو را به همان مقدار تجویز شده مصرف کنید و دور از دسترس اطفال نگهداری کنید. در صورت مصرف بیش از حد یا ناخواسته ی دارو حتما با مرکز کنترل مسمومیت تماس بگیرید.
عوارض شایع آمی تریپتیلین
در مدت درمان با این دارو ممکن است خشکی دهان را تجربه کنید که برای برطرف نمودن آن می توانید از آدامس یا آبنبات های بدون شکر استفاده نمایید.یبوست، خواب آلودگی، تاری دید و احساس گیجی و خستگی نیز از عوارض شایع این دارو می باشند که با یک رژیم غذایی حاوی وعده های غذا سبک و ساده و حاوی فیبر مناسب و با محدود کردن غذا های چرب و پر ادویه و مصرف به اندازه آب در شبانه روز این علائم را کاهش دهید.-در ابتدای درمان با این دارو ممکن است افکار ناراحت کننده ی شما از گذشته بیشتر شوند اما نگران نباشید. باید بدانید که این افکار ناشی از مصرف این دارو و شروع اثر بخشی آن می باشند و به مرور زمان برطرف می شوند.بهتر است در این موارد افکار خودرا با نزدیکان و یا مشاور خود درمیان بگذارید و آگاهانه نسبت به برطرف کردن آنها اقدام نمایید .مصرف الکل در مدت درمان با این دارو می تواند احتمال بروز عوارض ناشی از این دارو را افزایش دهد.-اگر برای اولین بار از این دارو استفاده می کنید ممکن است با تغییر وضعیت سریع از حالت نشسته به ایستاده سرگیجه مختصری ایجاد شود که به مرور برطرف می شود. این حالت طبیعی است. هنگام برخواستن ، به ارامی بلند شويد.در صورتی که مبتلا به دیابت هستید ،مصرف این دارو ممکن است باعث افزایش قندخون شما شود لذا قندخون خودرا به صورت منظم چک کنید و با پزشک خود درمورد آن صحبت کنید.

The name of your medicine is APO-Amitriptyline Tablets. It contains the active ingredient amitriptyline hydrochloride.

It is used to treat:

Ask your doctor if you have any questions about why this medicine has been prescribed for you. Your doctor may have prescribed this medicine for another reason.

This medicine is available only with a doctor’s prescription.

Amitriptyline belongs to a group of medicines called tricyclic antidepressants (TCAs). TCAs work by correcting the imbalance of certain chemicals in the brain. These chemicals, called amines, are involved in controlling mood. By correcting this imbalance, TCAs can help relieve the symptoms of depression.

There is no evidence that this medicine is addictive.

Amitriptyline should not be used in children and adolescents aged less than 18 years for the treatment of depression. The safety and efficacy have not been satisfactorily established.

If you are taking any of these you may need a different dose or you may need to take different medicines.

Other medicines not listed above may also interact with amitriptyline.

Follow carefully all directions given to you by your doctor. Their instructions may be different to the information in this leaflet.

Your doctor will tell you how much of this medicine you should take. This will depend on your condition and whether you are taking any other medicines.

For depression, it is recommended that the dose is started at a low level and increased gradually. When satisfactory improvement has been reached, the dose should be reduced to the lowest amount that will maintain relief of symptoms.

For bed wetting, the doses recommended are low compared with those used for depression.

Do not stop taking your medicine or change your dosage without first checking with your doctor.

Swallow the tablet(s) whole with a glass of water.

Amitriptyline may be taken as a single dose (e.g. at bedtime) or as divided doses (e.g. three times a day), your doctor will advise you when to take it.

Take your dose(s) at the same time each day, this will have the best effect and will also help you remember when to take it.

It does not matter if you take it before, with or after food.

Some people notice an improvement in their depressive symptoms after 3 or 4 days however it may take up to 4 weeks.

Most children respond to bed wetting treatment in the first few days.

Please note your doctor may ask you to continue taking amitriptyline even after your symptoms have improved. It is important you continue taking your medicine for as long as your doctor tells you.

Make sure you have enough to last over weekends and holidays.

If you take more than one dose a day and it is almost time to take your next dose, skip the missed dose and take your next dose at the usual time. Otherwise, take it as soon as you remember and then go back to taking your medicine as you would normally.

However, if you only take one dose a day at bedtime, ask your doctor for advice regarding missed doses, as taking it another time when you remember may mean you have difficulty waking up in the morning or experience drowsiness in the day.

Do not take a double dose to make up for missed doses.

This may increase the chance of you experiencing side effects.

If you have trouble remembering to take your medicine, ask your pharmacist for some hints to help you remember.

If you think that you or anyone else may have taken too much of this medicine, immediately telephone your doctor or the Poisons Information Centre (Tel: 13 11 26 in Australia) for advice. Alternatively, go to the Accident and Emergency department at your nearest hospital.

Do this even if there are no signs of discomfort or poisoning. You may need urgent medical attention.

Tell your doctor that you are taking this medicine if:

Your doctor may occasionally do tests to make sure the medicine is working and to prevent side effects. Go to your doctor regularly for a check-up.

Tell any other doctors, dentists and pharmacists who are treating you that you take this medicine.

Amitriptyline may cause dry mouth. Tell your doctor or dentist if your mouth continues to feel dry for more than 2 weeks. Continuing dryness of the mouth may increase the chance of dental disease, including tooth decay and gum disease.

Tell your doctor immediately if you have any suicidal thoughts or other mental/mood changes. Occasionally, the symptoms of depression may include thoughts of harming yourself or committing suicide. These symptoms may continue or get worse during the first one to two months of treatment until the full antidepressant effect of the medicine becomes apparent.

Contact your doctor or a mental health professional right away or go to the nearest hospital for treatment if you or someone you know is showing any of the following warning signs of suicide:

Be careful when driving or operating machinery until you know how this medicine affects you.

Amitriptyline may reduce your alertness, cause drowsiness or dizziness in some people. If you experience any of these symptoms, do not drive, operate machinery or do anything else that could be dangerous. For the same reasons, children should not ride a bike, climb trees or do anything else that could be dangerous if they are drowsy. Combining amitriptyline with alcohol can make you more drowsy or dizzy. Your doctor may suggest you avoid alcohol while being treated for depression. Be careful getting up from a sitting or lying position. Dizziness, light-headedness or fainting may occur, especially when you get up quickly. Getting up slowly may help.

Tell your doctor as soon as possible if you do not feel well while you are taking amitriptyline or if you have any questions or concerns.

Do not be alarmed by the following lists of side effects. You may not experience any of them. All medicines can have side effects. Sometimes they are serious but most of the time they are not.

If you experience any of the following, stop taking your medicine and contact your doctor immediately or go to the Accident and Emergency department at your nearest hospital.

Other side effects not listed above may occur in some patients.

If you think you are having an allergic reaction to amitriptyline, do not take any more of this medicine and tell your doctor immediately or go to the Accident and Emergency department at your nearest hospital.

Symptoms of an allergic reaction may include some or all of the following:

Keep your medicine in its original packaging until it is time to take it.

If you take your medicine out of its original packaging it may not keep well.

Keep your medicine in a cool dry place where the temperature will stay below 25°C.

Do not store your medicine, or any other medicine, in the bathroom or near a sink. Do not leave it on a window sill or in the car. Heat and dampness can destroy some medicines.

Keep this medicine where children cannot reach it.

A locked cupboard at least one-and-a-half metres above the ground is a good place to store medicines.

If your doctor tells you to stop taking this medicine or it has passed its expiry date, your pharmacist can dispose of the remaining medicine safely.

Amitriptyline is a prescription drug. It comes as a tablet you take by mouth.

Amitriptyline oral tablet is not available as a brand-name drug. It’s only available as a generic drug. Generic drugs typically cost less than brand-name drugs.

Amitriptyline is used to help relieve symptoms of depression.

Amitriptyline belongs to a class of drugs called tricyclic antidepressants. A class of drugs is a group of medications that work in a similar way. These drugs are often used to treat similar conditions.

Amitriptyline works on your central nervous system to increase the level of certain chemicals in your brain, which improves your depression.

Amitriptyline can cause dizziness and drowsiness during the first few hours after you take it. If you notice drowsiness while you take this drug, your doctor may have you take your dose at bedtime.

Amitriptyline can also cause other side effects.

The more common side effects of amitriptyline can include:

Taking certain drugs with amitriptyline may cause serious side effects. You should not take these drugs and amitriptyline at the same time. Examples of these drugs include:

Using an MAOI with amitriptyline can lead to seizures or even death. Do not take an MAOI within two weeks of stopping amitriptyline, unless told to do so by your doctor. Also, do not start taking amitriptyline if you stopped taking a MAOI in the last two weeks, unless told to do so by your doctor. If you aren’t sure whether any of the drugs you take is an MAOI, ask your doctor or pharmacist.

Taking amitriptyline with certain drugs raises your risk of negative effects. Examples of these drugs include:

It hasn’t been confirmed that amitriptyline is safe and effective for use in children younger than 17 years.

Senior dosage (ages 65 years and older)

The kidneys of older adults may not work as well as they used to. This can cause your body to process drugs more slowly. As a result, more of a drug stays in your body for a longer time. This raises your risk of side effects.

Your doctor may start you on a lowered dose or a different dosing schedule. This can help keep levels of this drug from building up too much in your body.

You might experience an initial worsening of your depression, thoughts of suicide, and behavioral changes when you first start taking amitriptyline. This risk may last until the drug starts working for you.

If you’ve been taking this medication for a long time, you shouldn’t stop taking it suddenly. Stopping it suddenly may cause side effects such as nausea, headache, and tiredness. Don’t stop taking this drug without talking to your doctor. They will tell you how to slowly lower your dosage over time.

Research has indicated that this type of medication can cause effects similar to those caused by drugs called anticholinergics. This can raise your risk of dementia.

This drug can cause a severe allergic reaction. Symptoms can include:

If you develop these symptoms, call 911 or go to the nearest emergency room.

Don’t take this drug again if you’ve ever had an allergic reaction to it. Taking it again could be fatal (cause death).

The use of drinks that contain alcohol while taking amitriptyline raises your risk of serious side effects, including extreme drowsiness.

For people with heart disorders: Taking this drug could cause heart problems, such as irregular heart rhythm, heart attack, and stroke. Do not take this drug if you’re recovering from a recent heart attack.

For people with bipolar disorder: Before starting you on treatment with antidepressants such as amitriptyline, it’s important for your doctor to check your risk of bipolar disorder. Your doctor should do this because a major depressive episode is usually the first symptom noticed in people with bipolar disorder. This drug should not be used in people with bipolar disorder.

For people with a history of seizures: Taking this drug raises your risk of seizures. If you have a history of seizures, your doctor will monitor you closely while you’re taking this drug. If you have a seizure while taking this drug, stop taking it and call your doctor right away.

For people with a history of glaucoma or increased eye pressure: Taking this drug could increase the pressure in your eyes. If you have a history of glaucoma or increased eye pressure, your doctor will monitor you closely while you’re taking this drug.

For pregnant women: Amitriptyline is a category C pregnancy drug. That means two things:

Talk to your doctor if you’re pregnant or planning to become pregnant. This drug should only be used if the potential benefit justifies the potential risk to the fetus.

For women who are breastfeeding: Amitriptyline passes into breast milk and may cause side effects in a child who is breastfed. Talk to your doctor if you breastfeed your child. You may need to decide whether to stop breastfeeding or stop taking this medication.

Amitriptyline oral tablet is used for long-term treatment. It comes with risks if you don’t take it as prescribed.

If you stop taking the drug suddenly or don’t take it at all: If you don’t take amitriptyline, your depression may worsen. If you stop taking this medication suddenly you may have withdrawal side effects such as nausea, headache, and tiredness.

If you miss doses or don’t take the drug on schedule: Your medication may not work as well or may stop working completely. For this drug to work well, a certain amount needs to be in your body at all times.

If you take too much: You could have dangerous levels of the drug in your body. Symptoms of an overdose of this drug can include:

If you think you’ve taken too much of this drug, call your doctor or seek guidance from the American Association of Poison Control Centers at 1-800-222-1222 or through their online tool. But if your symptoms are severe, call 911 or go to the nearest emergency room right away.

What to do if you miss a dose: Take your dose as soon as you remember. But if you remember just a few hours before your next scheduled dose, take only one dose. Never try to catch up by taking two doses at once. This could result in dangerous side effects.

How to tell if the drug is working: Over time you should notice an improvement in your symptoms of depression. This can take more than a month.

Keep these considerations in mind if your doctor prescribes amitriptyline for you.

A prescription for this medication is refillable. You should not need a new prescription for this medication to be refilled. Your doctor will write the number of refills authorized on your prescription.

When traveling with your medication:

Your doctor will monitor your mental health while you take this drug. Be sure to tell your doctor about any unusual changes in your behavior and mood.
