Celecoxib is a non-steroidal anti-inflammatory drug (NSAID) used in the treatment of osteoarthritis, rheumatoid arthritis, acute pain, painful menstruation and menstrual symptoms, and is also used to reduce numbers of colon and rectum polyps in patients with familial adenomatous polyposis. It is marketed by Pfizer under the brand name Celebrex. Celecoxib is available by prescription in capsule form 7.

By inhibiting prostaglandin synthesis, non-steroidal anti-inflammatory drugs (NSAIDs) cause mucosal damage, ulceration and ulcer complication throughout the gastrointestinal tract. Celecoxib, however, poses less of an ulceration risk, owing to its decreased effect on gastric mucosal prostaglandin synthesis, when compared to placebo 8.

Interestingly, selective COX-2 inhibitors (especially celecoxib), have been evaluated as potential cancer chemopreventive and therapeutic drugs in clinical trials for a variety of malignancies 7.

For the relief and management of osteoarthritis (OA), rheumatoid arthritis (RA), juvenile rheumatoid arthritis (JRA), ankylosing spondylitis, acute pain, primary dysmenorrhea and oral adjunct to usual care for patients with familial adenomatous polyposis Label.

Celecoxib, a selective cyclooxygenase-2 (COX-2) inhibitor, is classified as a nonsteroidal anti-inflammatory drug (NSAID).
The inhibition of prostaglandin E2 synthesis which (a mediator of pain) results from the inhibition of COX-2, and helps to alleviate pain symptoms Label.

Celecoxib is used to treat rheumatoid arthritis, osteoarthritis, and familial adenomatous polyposis (FAP). Due to its lack of platelet effects, celecoxib is not a substitute for aspirin for cardiovascular prophylaxis Label.

Unlike most NSAIDs, which inhibit both types of cyclooxygenases (COX-1 and COX-2), celecoxib is a selective noncompetitive inhibitor of cyclooxygenase-2 (COX-2) enzyme Label.

The mechanism of action of celecoxib is the inhibition of prostaglandin synthesis through COX-2 inhibition Label.

COX-2 is expressed heavily in inflamed tissues where it is induced by inflammatory mediators. COX-2 also plays physiological roles in a limited number of tissues, including those of the female reproductive tract, the kidney, and possibly the vascular endothelium 13.

When taken with a high-fat meal, peak plasma levels are delayed for about 1 to 2 hours with an increase in total absorption (AUC) of 10% to 20% Label.

A meta-analysis of pharmacokinetic studies has suggested an approximately 40% higher AUC (area under the curve) of celecoxib in black patients compared to Caucasians for unknown reasons 13.

The apparent volume of distribution at steady state (Vss/F) is about 400 L, suggesting extensive distribution into various tissues. Celecoxib is not preferentially bound to red blood cells Label.

Celecoxib metabolism is primarily mediated via cytochrome P450 2C9, in the liver. Three metabolites, a primary alcohol, the corresponding carboxylic acid and its glucuronide conjugate, have been identified in human plasma. These metabolites are inactive as COX-1 or COX-2 inhibitors. Patients who are known or suspected to be P450 2C9 poor metabolizers, based on a previous history, should be administered celecoxib with caution as they may have abnormally high plasma levels due to their reduced metabolic clearance Label.

When celecoxib capsules were taken with a high-fat meal, peak plasma levels were delayed for about 1 to 2 hours with an increase in total absorption (AUC) of 10% to 20% Label.

Celecoxib is eliminated predominantly by hepatic metabolism with little (Label.

The effective half-life is approximately 11 hours when a single 200 mg dose is given to healthy subjects. Terminal half-life is generally variable because of the low solubility of the drug thus prolonging absorption Label.

Apparent clearance (CL/F), single oral 200 mg dose, healthy subjects = 27.7 L/hr Label.

LD50 in both the rat and dog is >2000 mg/kg Label.

It is not advisable to administer celecoxib in patients with renal impairment or advanced hepatic impairment Label.
Symptoms of overdose celecoxib include breathing difficulties, coma, drowsiness, gastrointestinal bleeding, high blood pressure, kidney failure, nausea, sluggishness, stomach pain, and vomiting 11. Because serious GI tract ulcerations and bleeding can occur without preceding symptoms, physicians should monitor for signs/symptoms of GI bleeding Label.

Multiple studies have been performed on the coadministration of celecoxib and proton pump inhibitors 9, 10. It has been concluded that in patients at high risk of both cardiovascular and gastrointestinal events who require concomitant aspirin and NSAID, celecoxib in addition to a proton-pump inhibitor is the preferred treatment to reduce the risk of recurrent episodes of upper gastrointestinal bleeding 10.

A note on cardiovascular safety:

The concerns about the cardiovascular thrombotic risk of COX-2 selective NSAIDS emerged in the early 2000’s. Following an FDA Advisory Committee meeting in 2005, in which data from large clinical outcome trials in a wide range of indications and epidemiology studies of several individual NSAIDs were analyzed, the FDA concluded that the risk for cardiovascular thrombotic events was apparent for both COX-2 selective NSAIDs and nonselective NSAIDs 11.

Importantly, postmarketing cardiovascular outcomes trial (PRECISION) found that the lowest dose of celecoxib was similar to moderate doses of naproxen and ibuprofen in regards to cardiovascular (CV) safety 11. Patients with recent cardiovascular events such as acute MI, coronary revascularization, or coronary stent placement were not studied in the PRECISION trial. NSAID use is not advisable in these groups of patients 11.

داروی سلبرکس چیست و چه کاربردهایی دارد؟ در چه شرایطی و به چه صورت مصرف می شود؟ عوارض جانبی آن کدامند؟ چه داروهایی را نباید به طور همزمان با داروی سلبرکس مصرف کرد؟ این سوالات، به همراه نکات و اطلاعات مهم درباره‌ی این دارو، در مطلب پیش‌رو بررسی خواهد شد.

داروی سلبرکس (Celebrex)، که با نام عمومی سلکوکسیب (Celecoxib) نیز شناخته می‌شود، میزان هورمون هایی را که باعث التهاب و درد در بدن می‌شوند، کاهش می‌د‌هد. این دارو برای درمان التهاب و دردهایی که در اثر شرایط مختلف به وجود آمده‌اند- مثلاً ناشی از آرتریت، درد قاعدگی و غیره – استفاده می‌گردد. سلبرکس برای درمان آرتریت روماتویید در کودکان بالاتر از ۲ سال به کار می‌رود. این دارو ممکن است در موارد دیگری نیز جهت درمان استفاده شود.

سلبرکس می‌تواند باعث افزایش خطر سکته‌ی قلبی کُشنده یا سکته‌ی مغزی شود؛ به خصوص اگر به مدت طولانی و یا به میزان زیاد مصرف شود و یا در صورتی که شما دچار مشکلات و بیماری‌های قلبی باشید. از این دارو بلافاصله قبل یا بعد از جراحی بای پس قلبی، استفاده ننمائید.

سلبرکس می‌تواند باعث ایجاد خونریزی شدید معده یا روده در افراد شود؛ به طوری که کُشنده و بسیار خطرناک باشد. این وضعیت، وقتی در دوره مصرف این دارو هستید ممکن است بدون علامت خاصی اتفاق بیفتد و خصوصاً در افراد مسن‌تر، شایع‌تر است. بنابراین در صورتی که شما هم‌اکنون دچار خونریزی معده یا روده هستید، از این دارو استفاده نکنید.

یادآوری می‌شود که داروی سلبرکس می‌تواند باعث افزایش خطر سکته‌ی قلبی کُشنده یا سکته‌ی مغزی شود؛ به خصوص اگر به مدت طولانی و یا به میزان زیاد مصرف شود و یا در صورتی که شما دچار مشکلات و بیماری‌های قلبی باشید. حتی افرادی که بیماری‌های قلبی ندارند و یا عوامل خطرزا در آن‌ها نیست، هم ممکن است در دوره مصرف این دارو، دچار سکته‌ی قلبی یا مغزی شوند.

از این دارو بلافاصله قبل یا بعد از جراحی بای پس قلبی، استفاده ننمائید.

به دلیل اهمیت، دوباره یادآوری می‌شود که سلبرکس می‌تواند باعث ایجاد خونریزی شدید معده یا روده در افراد شود؛ به طوری که کُشنده و بسیار خطرناک باشد. این وضعیت، وقتی در دوره مصرف این دارو هستید ممکن است بدون علامت خاصی اتفاق بیفتد و خصوصاً در افراد مسن‌تر، شایع‌تر است.

در صورتی که نسبت به سلکوکسیب آلرژی (حساسیت) داشته و یا موارد زیر را دارید، از داروی سلبرکس استفاده نکنید:

برای اطمینان از اینکه مصرف این دارو برای شما خطراتی در پی ندارد، در صورتی که موارد زیر را دارید، به دکتر خود اطلاع دهید:

مصرف سلبرکس در سه ماهه‌ی پایانی بارداری ممکن است باعث صدمه به جنین گردد. بنابراین چنانچه در دوره مصرف این دارو باردار هستید و یا قصد باردار شدن را دارید، به پزشک خود اطلاع دهید.

سلکوکسیب می‌تواند در شیر مادر نفوذ پیدا کرده و باعث آسیب به نوزاد شود. اگر در دوران شیردهی قرار دارید، به دکتر خود اطلاع دهید.

مصرف سلبرکس برای افراد زیر ۲ سال ممنوع است.

سلبرکس را حتماً با تجویز پزشک مصرف کنید و قبل از مصرف، تمامی مواردی که روی جلد این دارو نوشته شده است را به دقت مطالعه نمایید. همچنین از مصرف دوز بیشتر یا کمتر از مقداری که برای شما تجویز شده است، خودداری کنید و فقط در همان دوره ی زمانی که برای شما تجویز شده، آن را مصرف کنید. کمترین میزانی از این دارو که می‌تواند به بهبود وضعیت شما کمک کند را مصرف کنید.

شما می‌توانید داروی سلبرکس را به همراه غذا یا بدون آن مصرف کنید. اگر استفاده از سلبرکس باعث می‌شود معده‌ی شما درد بگیرد، آن را به همراه غذا مصرف کنید.

قرص سلبرکس را به طور کامل قورت دهید و آن را نجوید.

اگر از این دارو به مدت طولانی می‌خواهید استفاده کنید، باید هر از گاهی وضعیت سلامت خود را با انجام آزمایشات بررسی کنید.

دارو را در دمای اتاق نگهداری کرده و دور از گرما و رطوبت قرار دهید.

همه‌ی اطلاعاتی که در مورد این دارو نوشته شده را به دقت مطالعه کنید و از دستورالعمل پزشک تبعیت نمائید و در صورتی که سوالی داشتید، از مسئول داروخانه یا دکتر خود بپرسید.

گاهی سلبرکس طبق یک برنامه‌ی زمانی مصرف نمی‌شود و هر موقع که نیاز بود، بیمار آن را استفاده می‌نماید. ولی هنگامی که این دارو را طبق یک برنامه‌ی زمانی مشخص مصرف می‌کنید، اگر یک بار مصرف آن را فراموش کردید، به محض اینکه به یاد آوردید، آن را مصرف کنید. اما چنانچه زمانِ به یادآوری شما، بسیار نزدیک به زمانِ مصرفِ بعدی بود، دوز فراموش شده را مصرف نکنید. فقط در نوبت بعدی، سلبرکس را مصرف کنید. از مصرف داروی اضافه، برای جبران دوز مصرفیِ فراموش شده، اجتناب کنید.

چنانچه بیشتر از میزان مورد نیاز، این دارو را مصرف کردید، به موارد اورژانسی در مورد مصرف زیاد این دارو مراجعه کنید و یا از پزشک کمک بگیرید.

از مصرف الکل اجتناب کنید. چرا که ممکن است باعث افزایش خطر خونریزی معده در شما شود.

از مصرف آسپیرین یا داروهای ضد التهابی غیر استروئیدی، در دوره مصرف سلبرکس خودداری کنید.

قبل از مصرف هر گونه داروی سرما خوردگی، حساسیت، یا مسکن‌ها، با دکتر خود مشورت کنید. بسیاری از داروهای موجود در داروخانه‌ها، حاوی آسپیرین یا داروهای مشابه با سلکوکسیب هستند؛ بنابراین مصرف همزمان آن‌ها باعث بالا رفتن دوز مصرف شما از این ماده می‌شود. برچسب اطلاعات داروها را قبل از مصرف آن ها به دقت مطالعه کنید تا مطمئن شوید حاوی آسپیرین، ایبوپروفن، کتوپروفن، یا ناپروکسن نیستند.

اگر هر گونه علائم آلرژیکی نسبت به این دارو – شامل کهیر، به دشواری نفس کشیدن، التهاب صورت، لب ها، زبان یا گلو – در خود مشاهده کردید، حتماً به دکتر اطلاع دهید.

در صورتی که علائم مربوط به سکته‌ی قلبی یا مغزی – شامل: درد قفسه‌ی سینه به طوری که تا قسمت فک و شانه امتداد بیابد، بی‌حسی یا ضعف ناگهانی در یک طرف بدن، لکنت زبان، احساس تنگی نفس – را در خود مشاهده کردید، از کمک‌های اورژانسی بهره ببرید.

اگر موارد زیر را دارید، بلافاصله مصرف دارو را قطع کرده و به دکتر اطلاع دهید:

البته لازم است ذکر شود که ممکن است عوارض جانبی دیگری نیز در صورت استفاده از این دارو رخ دهد. چنانچه موارد دیگری را مشاهده نمودید، به دکتر مراجعه کنید.

در صورتی که از یک داروی ضد افسردگی – مانند: سیتالوپرام، اسیلاتوپرام، فلوکستین (پروزاک)، فلووکسامین، پاروکستین، سرترالین (Zoloft)، ترازودون یا ویولوزدون – استفاده می‌کنید، قبل از مصرف سلبرکس دکتر خود را در جریان قرار دهید. مصرف هر یک از این داروها با داروهای ضد التهابی غیر استروئیدی (از جمله همین داروی سلبرکس) ممکن است باعث شود شما به راحتی دچار کبودی یا خونریزی شوید.

هر دارویی که مصرف می‌کنید یا قصد شروع یا قطع مصرف آن را دارید، با دکتر در میان گذارید؛ خصوصاً داروهای زیر:

البته لیست بالا همه‌ی داروهایی که با سلبرکس تداخل دارند را در بر ندارد و سایر داروها نیز ممکن است با این دارو تداخل داشته باشند؛ مثلاً ویتامین ها و داروهای گیاهی.

Celebrex is used to relieve the symptoms of joint pain, tenderness, swelling and stiffness in:

Celebrex also provides short-term pain relief in conditions such as:

Celebrex contains celecoxib and belongs to a group of medicines called coxibs which are used to relieve pain and inflammation in a number of conditions.

Although Celebrex can relieve the symptoms of pain and inflammation, it will not cure your condition.

Your doctor, however, may have prescribed Celebrex for another purpose.

Ask your doctor if you have any questions about why Celebrex has been prescribed for you.

Celebrex is not recommended for use in children or adolescents under 18 years of age.

This medicine is only available with a doctor’s prescription.

Do not take Celebrex if you

Symptoms of an allergic reaction to these medicines may include:

If you are allergic to sulfonamides or any of the capsule ingredients and take Celebrex, these symptoms may be severe.

Ask your doctor or pharmacist if any of this applies to you. Celebrex may not be suitable for you if any of the conditions below apply to you. You:

Do not take Celebrex if the expiry date printed on the packaging has passed, even though the capsules may look alright.
If you take this medicine after the expiry date has passed, it may not work as well.

Do not take Celebrex if the packaging is torn or shows signs of tampering.

If you are not sure if you should be taking Celebrex, talk to your doctor.

You must tell your doctor if you have any of the following health problems:

If you have not told your doctor or pharmacist about these things, tell them before you start taking Celebrex.

Tell your doctor or your pharmacist if you are taking any other medicines, including medicines you buy without a prescription from a pharmacy, supermarket or health food shop.

Some medicines and Celebrex may interfere with each other. These include:

Your doctor may need to adjust the dosage of these medicines, or provide additional advice if you are also taking Celebrex.

Follow all directions given to you by your doctor or pharmacist carefully.

Osteoarthritis

200 mg once daily or 100 mg twice daily.


Rheumatoid arthritis

100 mg twice daily.

Your doctor may increase the dose to 200 mg twice a day for a short period of time if you have a flare up.


Ankylosing spondylitis100 mg twice daily or 200 mg once daily.

Menstrual cramps or period pain

400 mg as a single dose on the first day and 200 mg twice daily on following days. You may take Celebrex for up to 5 days.

Muscle and joint injuries or after surgery

400 mg as a first dose followed by 200 mg once or twice daily as required. You may take Celebrex for up to 5 days.

Swallow the capsules whole with a full glass of water. Celebrex can be taken with or without food.

Take your medicine at about the same time each day.
Taking it at the same time each day will have the best effect. It will also help you remember when to take it.

If you need to take an antacid, take it at least 2 hours before or 2 hours after your dose of Celebrex

Depending on your condition, you may need Celebrex for a few days or for longer periods.

Celebrex will not cure your condition but should help control pain, swelling and stiffness.

Keep taking Celebrex for as long as your doctor advises.

Do not exceed the dose recommended by your doctor.

Your risk of developing heart or blood vessel diseases (e.g., heart attack) may increase with dose and duration of use even if you don’t have a history of heart or blood vessel disease.

If you need to take Celebrex for a long time see your doctor for regular check-ups so that he/she can monitor your condition and treatment.

If it is almost time for your next dose, skip the dose you missed and take your next dose when you are meant to. Otherwise, take it as soon as you remember, then go back to taking your capsules as you would normally.

Do not take a double dose to makeup for the dose you missed.

Immediately telephone your doctor or Poisons Information Centre (telephone 13 11 26) for advice, or go to Accident and Emergency at your nearest hospital if you think you or anyone else may have taken too much Celebrex. Do this even if there are no signs of discomfort or poisoning. You may need urgent medical attention.

If you take too much Celebrex, you may feel tired, drowsy, sick, vomit, and have stomach pain. You may also have difficulty breathing and feel faint.

If you become pregnant while taking Celebrex, tell your doctor immediately.

If you are about to start any new medicines, tell your doctor and pharmacist that you are taking Celebrex.

Tell all doctors, dentists and pharmacists who are treating you that you are taking Celebrex.

If you develop any skin rash (e.g. hives, spots) while being treated with Celebrex, contact your doctor immediately.
The onset of these events, if they occur, can occur at any time, but most often occur in the first month of treatment.

If you are about to have any blood tests, tell your doctor that you are taking this medicine.

Do not give Celebrex to anyone else, even if they have the same symptoms or condition as you.

Do not take Celebrex to treat any other complaints unless your doctor tells you to.

Check with your doctor as soon as possible if you have any problems while taking Celebrex, even if you do not think the problems are connected with the medicine or are not listed in this leaflet.

Like other medicines, Celebrex can cause some side effects. If they occur, most are likely to be minor and temporary.

Ask your doctor or pharmacist any questions you may have.

A locked cupboard at least one-and-a-half metres above the ground is a good place to store medicines.

Keep Celebrex in a cool, dry place where the temperature stays at or below 25°C. Do not store it, or any other medicine, in the bathroom or near a sink. Do not leave it in the car or on window sills.
Heat and dampness can destroy some medicines.

Keep your capsules in their blister pack until it is time to take them.
If you take the capsules out of their container they may not keep well.

If your doctor tells you to stop taking Celebrex or the expiry date has passed, ask your pharmacist what to do with any medicine that is left over.

Celecoxib, sold under the brand name Celebrex among others, is a COX-2 selective nonsteroidal anti-inflammatory drug (NSAID).[2] It is used to treat the pain and inflammation in osteoarthritis, acute pain in adults, rheumatoid arthritis, ankylosing spondylitis, painful menstruation, and juvenile rheumatoid arthritis.[2] It may also be used to decrease the risk of colorectal adenomas in people with familial adenomatous polyposis.[2] It is taken by mouth.[2] Benefits are typically seen within an hour.[2]

Common side effects include abdominal pain, nausea, and diarrhea.[2] Serious side effects may include heart attacks, strokes, gastrointestinal perforation, gastrointestinal bleeding, kidney failure, and anaphylaxis.[3][2] Use is not recommended in people at high risk for heart disease.[4][5] The risks are similar to other NSAIDs, such as ibuprofen and naproxen.[6] Use in the later part of pregnancy or during breastfeeding is not recommended.[2]

Celecoxib was patented in 1993 and came into medical use in 1999.[7] It is available as a generic medication.[8] A month supply in the United Kingdom costs the NHS less than £2 as of 2019.[8] In the United States the wholesale cost of this amount is about $US12.90.[9] In 2016 it was the 120th most prescribed medication in the United States with more than 6 million prescriptions.[10] In 2011 it was one of Pfizer’s best-selling medications, with $2.5 billion in sales.[11]

Celecoxib is used for osteoarthritis, rheumatoid arthritis, acute pain, musculoskeletal pain, painful menstruation, ankylosing spondylitis, and to reduce the number of colon and rectal polyps in people with familial adenomatous polyposis.[12] It may also be used in children with juvenile rheumatoid arthritis who are older than two years of age and weigh more than 10 kg (22 lb).[12]

For postoperative pain, it is more or less equal to ibuprofen.[13] For pain relief, it is similar to paracetamol (acetaminophen).[14] And in osteoarthritis, acetaminophen is the first line treatment.[15][16] In knee and hip osteoarthritis, acetaminophen may be ineffective.[17]

Evidence of effects are not clear as a number of studies done by the manufacturer have not been released for independent analysis.[18]

Tentative evidence supports its use in treating a number of psychiatric disorders, including major depression, bipolar disorder, and schizophrenia.[19][20][21][22]

It has been used to reduce colon and rectal polyps in people with familial adenomatous polyposis, but it is not known if it decreases rates of cancer,[23] so it is not a good choice for this reason.[23]

People with prior history of ulcer disease or GI bleeding require special precaution. Moderate to severe liver impairment or GI toxicity can occur with or without warning symptoms in people treated with NSAIDs.

Celecoxib contains a sulfonamide moiety and may cause allergic reactions in those allergic to other sulfonamide-containing drugs. This is in addition to the contraindication in people with severe allergies to other NSAIDs. However, it has a low (reportedly 4%) chance of inducing cutaneous reactions among persons who have a history of such reactions to aspirin or nonselective NSAIDs. NSAIDs may cause serious skin adverse events, including exfoliative dermatitis, Stevens-Johnson syndrome, and toxic epidermal necrolysis; events may occur without warning and in patients without prior known sulfa allergy. Use should be discontinued at first sign of rash (or any other hypersensitivity).

The coxibs (which includes celecoxib) increase the risk of major cardiovascular problems by about 37% over placebo, according to a 2013 meta-analysis of hundreds of trials.[3]

The COX-2 inhibitor rofecoxib (Vioxx) was removed from the market in 2004 due to its risk. Like all NSAIDs on the US market, celecoxib carries an FDA-mandated “black box warning” for cardiovascular and gastrointestinal risk. In February 2007, the American Heart Association warned that with respect to “patients with a prior history of or at high risk for cardiovascular disease… use of COX-2 inhibitors for pain relief should be limited to patients for whom there are no appropriate alternatives, and then, only in the lowest dose and for the shortest duration necessary.”[4]

In 2005, a study published in the Annals of Internal Medicine found that cardiovascular effects of COX-2 inhibitors differ, depending on the drug.[26] Other COX-2-selective inhibitors, such as rofecoxib, have significantly higher myocardial infarction rates than celecoxib.[27] In April 2005, after an extensive review of data, the FDA concluded it was likely “that there is a ‘class effect’ for increased CV risk for all NSAIDs”.[28] In a 2006 meta-analysis of randomized control studies, the cerebrovascular events associated with COX-2 inhibitors were examined, but no significant risks were found when compared to nonselective NSAIDs or placebos.[29]

In 2016, a randomized trial involving more than 24,000 participants provided strong evidence that treatment with celecoxib is not more likely to result in poor cardiovascular outcomes than treatment with naproxen or ibuprofen.[30] As a result, in 2018 an FDA advisory panel concluded that celecoxib poses no greater risk for causing heart attacks and strokes than the commonly-used NSAIDs ibuprofen or naproxen and recommended that the FDA consider changing its advice to physicians regarding celecoxib’s safety.[6]

Celecoxib is predominantly metabolized by cytochrome P450 2C9. Caution must be exercised with concomitant use of 2C9 inhibitors, such as fluconazole, which can greatly elevate celecoxib serum levels.[25] If used concomitantly with lithium, celecoxib increases lithium plasma levels.[25] If used concomitantly with warfarin, celecoxib may result in increased risk of bleeding complications.[25] The drug may increase the risk of kidney failure with angiotensin-converting enzyme-inhibitors, such as lisinopril, and diuretics, such as hydrochlorothiazide.[31]

In the US FDA’s pregnancy categories, the drug is category C prior to 30 weeks gestation, and category D starting at 30 weeks gestation.[31]

A highly selective reversible inhibitor of the COX-2 isoform of cyclooxygenase, celecoxib inhibits the transformation of arachidonic acid to prostaglandin precursors. Therefore, it has antipyretic, analgesic and anti-inflammatory properties.[25] Nonselective NSAIDs (such as aspirin, naproxen, and ibuprofen) inhibit both COX-1 and COX-2. Inhibition of COX-1 (which celecoxib does not inhibit at therapeutic concentrations) inhibits the production of prostaglandins and the production of thromboxane A2, a platelet activator.[25] COX-1 is traditionally defined as a constitutively expressed “housekeeping” enzyme and plays a role in the protection of the gastrointestinal mucosa, kidney hemodynamics, and platelet thrombogenesis.[32][33] COX-2, on the contrary, is extensively expressed in cells involved in inflammation and is upregulated by bacterial lipopolysaccharides, cytokines, growth factors, and tumor promoters.[32][34] Celecoxib is approximately 10-20 times more selective for COX-2 inhibition over COX-1.[33][35] It binds with its polar sulfonamide side chain to a hydrophilic side pocket region close to the active COX-2 binding site.[36] In theory, this selectivity allows celecoxib and other COX-2 inhibitors to reduce inflammation (and pain) while minimizing gastrointestinal adverse drug reactions (e.g. stomach ulcers) that are common with nonselective NSAIDs.[37]

For its use in reducing colon polyps, celecoxib affects genes and pathways involved in inflammation and malignant transformation in tumors, but not normal tissues.[38]

Celecoxib binds to Cadherin-11 (which may explain the reduction in cancer progression).[39]

The Searle research group found the two appropriately substituted aromatic rings must reside on adjacent positions about the central ring for adequate COX-2 inhibition. Various modifications can be made to the 1,5-diarylpyrazole moiety to deduce the structure-activity relationship of celecoxib.[40] A para-sulfamoylphenyl at position 1 of the pyrazole was found to have a higher potency for COX-2 selective inhibition than a para-methoxyphenyl (see structures 1 and 2, below). In addition, a 4-(methylsulfonyl)phenyl or 4-sulfamoylphenyl is known to be necessary for COX-2 inhibition. For instance, replacing either of these entities with a –SO2NHCH3 substituent diminishes COX-2 inhibitory activity as noted with a very high inhibitory concentration-50 (see structures 3 – 5). At the 3-position of the pyrazole, a trifluoromethyl or difluoromethyl provides superior selectivity and potency compared to a fluoromethyl or methyl substitution (see structures 6 – 9).[40]

Celecoxib is compound 22; the 4-sulfamoylphenyl on the 1-pyrazol substituent is required for COX-2 inhibition and the 4-methyl on the 5-pyrazol system has low steric hindrance to maximize potency, while the 3-trifluoromethyl group provides superior selectivity and potency.[40] To explain the selectivity of celecoxib, it is necessary to analyze the free energy of binding difference between the drug molecule and COX-1 compared to COX-2 enzymes. The structural modifications highlight the importance of binding to residue 523 in the side binding pocket of the cyclooxygenase enzyme, which is an isoleucine in COX-1 and a valine in COX-2.[41] This mutation appears to contribute to COX-2 selectivity by creating steric hindrance between the sulfonamide oxygen and the methyl group of Ile523 that effectively destabilizes the celecoxib-COX-1 complex.[41] Thus, it is reasonable to expect COX-2-selective inhibitors to be more bulky than nonselective NSAIDs.

It was initially marketed by Pfizer for arthritis. Celecoxib and other COX-2 selective inhibitors, valdecoxib, parecoxib, and mavacoxib, were discovered by a team at the Searle division of Monsanto led by John Talley.[42][43]

After the withdrawal of rofecoxib from the market in September 2004, celecoxib enjoyed a robust increase in sales. However, the results of the APC trial in December of that year raised concerns that Celebrex might carry risks similar to those of rofecoxib, and Pfizer announced a moratorium on direct-to-consumer advertising of Celebrex soon afterwards. Sales reached $2 billion in 2006.[4] Prior to its availability in generic form, it was one of Pfizer’s “best-selling drugs, amounting to more than $2.5 billion in sales [by 2012], and was prescribed to 2.4 million” people in 2011.[11] By 2012, 33 million Americans had taken celecoxib.[11]

Pfizer resumed advertising Celebrex in magazines in 2006,[54] and resumed television advertising in April 2007 with an unorthodox, ​2 1⁄2-minute advertisement which extensively discussed the adverse effects of Celebrex in comparison with other anti-inflammatory drugs. The ad drew criticism from the consumer advocacy group Public Citizen, which called the ad’s comparisons misleading.[55] Pfizer responded to Public Citizen’s concerns with assurances that they are truthfully advertising the risk and benefits of Celebrex as set forth by the FDA.[55]

In late 2007, Pfizer released another US television ad for Celebrex, which also discussed celecoxib’s adverse effects in comparison with those of other anti-inflammatory drugs.

Pfizer and its partner, Pharmacia presented findings from their study that Celebrex was “better in protecting the stomach from serious complications than other drugs.”[11] This became Celebrex’s main selling point. However, following federal investigations it was revealed that Pfizer and Pharmacia “only presented the results from the first six months of a year long study rather than the whole thing.” These partial results were then published in The Journal of the American Medical Association.[11] In 2001 Food and Drug Administration released the full results of the Pfizer and Pharmacia study which showed that they had withheld crucial data.[11] By 2012 a federal judge unsealed “thousands of pages of internal documents and depositions” in a “long-running securities fraud case against Pfizer.”[11]

On March 11, 2009, Scott S. Reuben, former chief of acute pain at Baystate Medical Center, Springfield, Massachusetts, revealed that the data for 21 studies he had authored for the efficacy of the drug (along with others such as Vioxx) had been fabricated. The analgesic effects of the drugs had been exaggerated. Reuben was also a former paid spokesperson for Pfizer. Although from 2002 to 2007 Pfizer underwrote much of Dr. Reuben’s research and “many of his trials found that Celebrex and Lyrica, Pfizer drugs, were effective against postoperative pain,” Pfizer was not aware of the fraudulent data.[56][not in citation given] None of the retracted studies were submitted to either the US Food and Drug Administration or the European Union’s regulatory agencies prior to the drug’s approval. Although Pfizer issued a public statement declaring, “It is very disappointing to learn about Dr. Scott Reuben’s alleged actions. When we decided to support Dr. Reuben’s research, he worked for a credible academic medical center and appeared to be a reputable investigator”,[57][58] the documents unsealed in 2012 revealed that by February 2000, Pharmacia employees had devised a strategy to present the findings.[11]

Pfizer markets celecoxib under the brand name Celebrex, and it is available as oral capsules containing 50, 100, 200 or 400 mg of celecoxib.[59]

It is legally available in many jurisdictions as a generic under several brand names.[60] In the US, celecoxib was covered by three patents, two of which expired May 30, 2014, and one of which (US RE44048[61]) was due to expire December 2, 2015. On March 13, 2014, that patent was found to be invalid for double patenting.[62] Upon the patent expiry on May 30, 2014, FDA approved the first versions of celecoxib generic.[63]

The role celecoxib might have in reducing the rates of certain cancers has been the subject of many studies. However, no current medical recommendation exists to use this drug for cancer reduction.

The use of celecoxib to reduce the risk of colorectal cancer has been investigated, but neither celecoxib nor any other drug is indicated for this use.[64] Small-scale clinical trials in very high-risk people (belonging to FAP families) showed celecoxib can prevent polyp growth. Hence, large-scale randomized clinical trials were undertaken.[65] Results show a 33 to 45% polyp recurrence reduction in people treated with celecoxib each day. However, serious cardiovascular events were significantly more frequent in the celecoxib-treated groups. Aspirin shows a similar (and possibly larger) protective effect,[66][67][68] has demonstrated cardioprotective effects and is significantly cheaper, but no head-to-head clinical trials have compared the two drugs.

Different from cancer prevention, cancer treatment is focused on the therapy of tumors that have already formed and have established themselves inside the patient. Many studies are going on to determine whether celecoxib might be useful for this latter condition.[69] However, during molecular studies in the laboratory, it became apparent that celecoxib could interact with other intracellular components besides its most famous target, COX-2. The discovery of these additional targets has generated much controversy, and the initial assumption that celecoxib reduces tumor growth primarily by the inhibition of COX-2 became contentious.[70]

Certainly, the inhibition of COX-2 is paramount for the anti-inflammatory and analgesic function of celecoxib. However, whether inhibition of COX-2 also plays a dominant role in this drug’s anticancer effects is unclear. For example, a recent study with malignant tumor cells showed celecoxib could inhibit the growth of these cells in vitro, but COX-2 played no role in this outcome; even more strikingly, the anticancer effects of celecoxib were also obtained with the use of cancer cell types that do not even contain COX-2.[71]

Additional support for the idea that other targets besides COX-2 are important for celecoxib’s anticancer effects has come from studies with chemically modified versions of celecoxib. Several dozen analogs of celecoxib were generated with small alterations in their chemical structures.[72] Some of these analogs retained COX-2 inhibitory activity, whereas many others did not. However, when the ability of all these compounds to kill tumor cells in cell culture was investigated, the antitumor potency did not at all depend on whether or not the respective compound could inhibit COX-2, showing the inhibition of COX-2 was not required for the anticancer effects.[72][73] One of these compounds, 2,5-dimethyl-celecoxib, which entirely lacks the ability to inhibit COX-2, actually displayed stronger anticancer activity than celecoxib.[74]

سلکوکسیب از مهارکننده‌های قوی آنزیم سیکلواکسیژناز است.آنزیم سیکلواکسیژناز باعث تبدیل اسید آراشیدونیک به پروستاگلاندین‌ها می‌شود . حداقل دو ایزوفرم سیکلواکسیژناز وجود دارد: Cox-۱ و Cox-۲ . تفاوت سلکوکسیب با سایر مهارکننده‌های سیکلواکسیژناز در این است که سلکوکسیب به صورت اختصاصی Cox-۲ را که در سلول‌های التهابی است مهار می‌کند و به Cox-۱ که در سایرسلول‌ها مانند سلول‌های معده است تأثیری ندارد. این دارو با مهار سیکلواکسیژناز تولید پروستاگلاندین‌ها را کاهش می‌دهد و به این روش اثر ضد درد و التهاب خود را نشان می‌دهد.

آنزیم کوکس۱ در تمام ارگان‌ها مانند معده و کبد و غیره پاسخگوی نیاز ایمنی در برابر آسیب‌ها بوده ولی آنزیم کوکس۲ تنها در محل بروز التهاب وجود دارد. ضد التهاب‌های کلاسیک مانند بروفن با بلوک کردن هردونوع آنزیم٬ در کنار مهار التهاب پی‌آمدهای ناخواسته چون آسیب‌های گوارشی و غیره را نیز همراه دارند٬ حال‌آن‌که داروهای کوکسیب به‌خاطر شکل کوکس۲ با بلوک کردن آن باعث رفع التهاب می‌شوند ولی قادر به بلوک کردن کوکس۱ نیستند.

کوکسیب(از سرنام COX-2 inhibitor) نسل نوین داروهای ضدالتهاب‌غیراستروئید انتخابی(سلکتیو) هستند که بر خلاف انواع سنتی مانند آسپیرین و بروفن با بازداشتن انتخابی Cox-۲ و بدون بلوک کردن آنزیم اسیکلوکسیژناز نوع یک (COX-1) عوارض گوارشی کمتری ایجاد می‌کنند. این رده دارو هنوز به‌طور کامل مورد استفاده قرار نگرفته و گمان می‌رود که عوارض کاردیوپاتی(قلبی) و کلیوی حادتری از انواع قدیمی ضد التهاب‌ها داشته باشد. داروی روفیوکوکسیب به همین دلیل از سال ۲۰۰۴ از فهرست داروهای داروخانه‌ها حذف شد.[۱][۲]

این دارو هرچند عوارض گوارشی کمتری از مهارکننده‌های غیراختصاصی سیکلواکسیژنار مانند دیکلوفناک و آسپرین دارد ولی عوارض کلیوی آن‌ها مشابه مهارکننده‌های غیراختصاصی Cox-۲ بوده و عوارض قلبی عروقی آن حتی ممکن است بیشتر باشد. واکنش‌های‌آلرژیک .همچنین در مواردی گزارش شده که مصرف این دارو در برخی افراد باعث بالا رفتن تپش قلب شده‌است.
