RATIONALE: Gastroesophageal reflux disease (GERD) is common in patients with
asthma, suggesting an interaction between the two conditions.
OBJECTIVES: To assess the effect of gastric acid suppression with the proton pump
inhibitor esomeprazole on asthma outcomes in subjects with persistent moderate to
severe asthma treated with antiinflammatory asthma medication.
METHODS: In this double-blind study, subjects were randomized to receive
esomeprazole 40 mg or placebo twice daily for 16 wk. According to nocturnal
respiratory symptoms (NOC) and GERD, subjects were divided into three strata:
GERD-/NOC+, GERD+/NOC-, and GERD+/NOC+.
MEASUREMENTS AND MAIN RESULTS: A total of 770 subjects were randomized. There was
no statistically significant improvement in morning peak expiratory flow (PEF)
over placebo in the overall study population: 6.3 L/min (p = 0.061). Over the
whole treatment period, in GERD+/NOC+ subjects (n = 350), esomeprazole provided
an 8.7-L/min improvement (p = 0.03) in morning PEF, and a 10.2-L/min improvement
(p = 0.012) in evening PEF over placebo. Among 307 subjects taking long-acting
beta2-agonists, improvements over placebo were observed in morning PEF (12.2
L/min, p = 0.017) and in evening PEF (11.1 L/min, p = 0.024); these improvements
were more pronounced in GERD+/NOC+ subjects. Esomeprazole 40 mg twice daily was
well tolerated and no safety concerns were noted.
CONCLUSIONS: Esomeprazole improved PEF in subjects with asthma who presented with
both GERD and NOC. In subjects without both GERD and NOC, no improvement could be
detected.

BACKGROUND: Symptoms of gastro-oesophageal reflux disease (GERD) may persist
despite daily treatment with a proton pump inhibitor (PPI).
AIM: To compare the pharmacodynamic effect of various esomeprazole dosage and
timing regimens in healthy volunteers.
METHODS: The effect of different esomeprazole dosage regimens [20 mg once daily
(od) before breakfast or dinner; 20 mg twice daily (b.d.); 40 mg od before
breakfast, dinner or at bedtime; and 40 mg b.d.] on 24-h, daytime and night-time
acid inhibition was evaluated in a randomized, seven-way crossover study in
healthy volunteers. Each regimen was taken for 5 days.
RESULTS: Over the 24-h period (day 5), esomeprazole 20 mg b.d. was associated
with superior acid inhibition vs. all 20 mg and 40 mg od regimens (P < 0.05), but
was less effective than esomeprazole 40 mg b.d. (P < 0.05). Dosing with
esomeprazole 20 mg or 40 mg od before breakfast gave improved 24-h and daytime
acid inhibition vs. the corresponding administration before dinner or at bedtime
(all P < 0.05). Night-time acid inhibition was improved when esomeprazole 40 mg
od was administered before dinner or at bedtime vs. before-breakfast dosing (P <
0.05).
CONCLUSION: Varying the dose and timing of esomeprazole administration may
provide acid inhibition appropriate for the symptom pattern of individual
patients with GERD.

I developed acid and/or bile reflux two months after gall bladder surgery. The Dr. put me on 40mg of Nexium for two weeks and I got about 70-80% relief of my symptoms. I still felt some burning in my throat and a lot of burping. The Dr increased the Nexium dosage to 80mg daily. I am concerned about side effects especially possible damage to my liver since I also take 40 mg of Lipitor daily.
I take Nexium for some time now. I found the more I burp the worse the acid reflex gets. So watch what you eat.
to whom it may concern, this may sound crazy but try a table spoon of vinegar before you feel an attack coming on. i take two table spoons and i swear by it.
Apple cider helps too.
Its not the best thing to do but my husband takes nexium 40 mg and he takes it in the morning but sometimes at night he fills the burn so he will take 1 more it has never cause any problems.and he does that about 6 to 8 time a month,hope this helps

I used to take Nexium 3 times a day for severe Acid Reflux and I don’t remember having any side effects… just made it a lot less painful. Don’t know if that’s helpful but I thought I’d share 🙂
Yes your answer is helpful, thanks. Could I ask you how long this 3 times a day treatment lasted? Did it get rid of your reflux permanently or did it just lessen the symptoms?
Honestly I don’t really remember but I think around a year. It definitely did not rid me of the reflux but it did take the majority of the pain away.
Funny how you are asking this question. I went to the doctor today and am having pain right below the ribcage. I have a history of reflux also. I usually take Nexium 40mg once a day. He suggested that I take 2 a day. I guess it is o.k. I was a little concern about long side effects too. Hope this helps you. Take care!
I’ve been taking 40mg of nexium twice a day for over a year and no big side effects. Last year I was experiencing pretty bad chest pain (mostly center of chest and just below rib cage) and wasn’t sure if it was my heart or something else. Had all the heart test done and no probs there so we are pretty sure it is from my hiatal hernia (often called the great mimic for mimicing a heart attack). One of my doctors said she had been to a seminar and saw that Nortriptyline has been used effectively to treat chest pain from hiatal hernia. I started taking a 10mg capsule once at night and this has pretty much eliminated my chest pain.

Your concern of side effects is valid but I would be more concerned about the effects on my bone health. I was taking the same dosage as you are and did for 12 years. On the other hand, you are going to have to really watch what you eat, the gall bladder issue you have is creating problems with you system being able to digest fats of any sort, remember some are supposed to be good, but not to your system anymore. If you can control the foods you eat and learn what is okay and not okay for your system, you will find that you will be less dependant on the nexium. Good health to you
